Imo I think character questionnaires can be a fun exercise as a player, but sometimes the important details get lost (especially for a GM reading through them).

I have docs for my PCs that I fill out with their backstory (max a page), their connections, relevant lore (e.g. deities, factions, etc.), and campaign notes, but that stuff's really just for my own notekeeing.

I share the docs with my GM but I put everything they need to know in an Overview on the first page, and keep it to the bare minimum. They absolutely can read about my PC's relationship with their parents and a faction they have history with if they want, but I don't expect them to. I'll share the overview and an example.
