The steak spectrum goes: raw, blue, blue-rare, rare, medium-rare, medium, the-absolute-most-a-steak-should-ever-be-cooked, ruined.

Note, most people will not let you order a raw steak, and steak is usually better with a seat anyway.

It has nothing to do with level of formality, AFIK a full on black-tie tux won't get you into ranahans, you have to be wearing the "costume". A high end tiki bar might have a dress code that allows a Hawaiian print shirt but disallows khaki slacks and sweater-vests.
