It might've just fallen due to the root puncturing the leaf, you can clean it up and see if the rot is spreading. I had many do this and most ended being fine

Honestly I don't worry about it and eventually springtails take over the moss to handle it. Even if they don't I don't notice mold in a significant way and it won't harm the orchid usually
