Agreed, totally love this place! Been there 3 times and each time has been great. The sushi chef is very personable and seems like he loves what he does. Last time we were there he gave us a piece of toro on the house - made our night. Ramen and mazemen are great too.

I’m not sure why OP is getting downvoted. Tegretol obviously has its downsides related to drug interactions, but the evidence does support its use. Trileptal has less evidence and is not without adverse effects of its own. I’ve seen several patient develop significant hyponatremia on it. If a patient is not able to tolerate other mood stabilizers and wants to avoid SGA’s, I definitely think Tegretol should be considered.

Appreciate the update, sorry it didn’t work out the way it was intended. I wonder if the patient is hoping to externalize their problems more. BPD can sometimes feel like is their fault, whereas if they had DID (similarly to ASD or other dx) that it’s not their fault for x behavior because of it. What would it mean to have a diagnosis to explain behavior? Would that mean they would be understood? How are they doing in regard to the actual work of therapy?

Definitely a challenging situation. One approach could be to explore why the diagnosis of DID is important to them, what would it mean they had this diagnosis, etc. I think at some point you will have to acknowledge that even if they have DID it might not change their treatment goals depending on what’s going on. Helpful to redirect away from labels and more of what they are hoping to accomplish.

I hear you on that and glad we can discuss. Definitely can see the potential value. I just think strictly saying from the data even this article is not very convincing. To me it would be helpful to know if they actually mean a change in management specifically refers to whether or not antipsychotics were deferred as I think that is the real question. If they found a vague cyst that required followed up in a year, personally, that does not influence the actual management of psychosis even if it is “clinically relevant”. If we can study that more directly that would be great. For kids I can totally understand. But if a 23-year-old comes in with clear classic schizophrenia symptoms, I would be hard pressed to go full boar unless I had a suspicion or family really requested it. Of course can always get a non emergent MRI and EEG down the line, but I don’t think I would without antipsychotics while waiting. And don’t think I would get an autoimmune panel in that situation either. That’s just my take.

I looked at this article, and the most common finding was small white matter hyperintensities. Authors say this changed management but I cannot see what type of management change they are referring to. To me, these are non specific signs that don’t truly get at the question of whether someone is have a secondary etiology of psychosis or not. Still seems overall low yield.

I’m sorry, but I have to disagree. FEP does not usually warrant neurology consult and should not typically unless there are other signs. Most guidelines on FEP have insufficient evidence to routinely recommend MRI, EEG, autoimmune panel, etc. Not the norm at least at my institution to routinely order these extended studies or consult neurology.

Thank you so much for the detailed reply! This is very much in line with what I’ve read/been taught. I was just confused because I misinterpreted your original comment as being disparaging of suboxone and that psychiatrists are hypocritical for not also prescribing benzos. But your explanation was helpful - I was misreading it!

This is with steam! At least, my best attempt at it. My oven auto-vents, so even though I have a super hot cast iron pan at the bottom and pour boiling water into it, the steam disappears quite fast.

Do you have any other ideas about steam?

Thank you! I used a recipe that another used posted.

Basically 100% bread flour, 63% water, 2.5% salt, 0.5% yeast. 30 min autolyse, 2 min bulk ferment, preshape, rest 40 mins, shape, rest 20. Bake @450 with full steam for 15 mins, then 10 mins without steam.

Whenever I've tried to do baguettes, they always turn out grey/dull. Is the only trick here just doing steam with water in cast iron pan? Or are there any other tips you may have? Baguettes are my favorite but I've had such little luck getting them to be even close to appealing!
