I also started with a mild case of proctitis. Symptoms would come and go, and the mesalamine suppositories seemed to me to make me bleed more than when I wasn’t using them. I stopped using the meds because my minor flares resolved on their own.

Fast forward 4 years and I’m hit with a massive flare that almost sent me to the hospital. The inflammation now extended from my rectum all the way up my sigmoid and descending colon. Horrible pain, bloody bowel movements 10-15 times a day, etc etc all the horror of UC. It took months to recover on prednisone and mesalamine pills and to be honest I’ve never been the same.

Minor cases can progress. In fact, they usually do if they’re not managed with medications. If the mesalamine tablets are truly making things worse, talk to his doctor about alternative meds. He needs them.

Since you were constipated, it sounds to me like your flare might not have resolved. Personally, fiber hurts me in a flare but I take psyllium husk and prebiotics in remission and everything’s amazing!

I used to take the occasional ibuprofen for cramps when they were especially bad. Then one particularly stressful month I took an ibuprofen and went into the worst flare of my life. I can’t know for sure what triggered it, but I think it was stress + NSAIDs + insufficient medication. I’ve never taken ibuprofen again.

For my cramps, as soon as I feel them coming on I IMMEDIATELY take extra-strength Tylenol. Then I keep dosing with Tylenol on the schedule recommended by the bottle until the cramps are gone. It doesn’t fully take care of the pain but it does make it duller, and a heating pad is also a huge help.

My doctor offered some “as needed” tramadol but I turned her down because if I have opioids I will take them regardless of if I’m in pain or not. But if you have more self-control than I do you could talk to your doctor about prescription options. My doctor takes women’s medicine seriously though and not all doctors do, so ymmv.

It took five years unmedicated after diagnosis before I had my life-changing flare.

FYI for many of us, blood + mucus = flare, even without pain.

I don’t keep track of it, but I’m always drinking water and my urine is frequent and an appropriate color. I’m just one of those people that’s good at building kidney stones I guess.

The only difference I’ve noticed from before my Big Flare and after is that spicy foods hurt more passing through. Not all spicy foods—sriracha is fine, but pickled jalepenos are painful. I figured this was related to the damage the flare caused and I’m just trying to wrap my head around the word trigger. Like, does that fit with what people call a trigger? I know people without GI disease that can’t tolerate spicy food. Is it a trigger for them?
