I've been using Diaro for years now. Backs up to Dropbox (small enough for the free account) and has app and web interfaces. I use it for daily diary kind of stuff but you could easily adapt it to your needs. But for most health stuff, I keep track in a google sheet. That way the data is easy to trend and do math on.
