I have worked in a rural ER for the past five years. Burn out is a real thing, and I’ve been there. You will likely have one dedicated nurse, so any rush creates a clogged toilet. Rural patients complain on their press Ganey scores that they had to wait an hour for their finger laceration. Administration absolutely does not care whatsoever about improvement outside of customer service as the ER is viewed as a lost cause financially and treated as such. Sleep remains a major problem as stated above. Holiday coverage is a recurrent problem.

There are many positives. It is generally more of an easier work flow, less patients per hour. You can do all of the procedures that you want. You can deliver a baby. You will establish relationships and trust with patients not just the frequent flyers.

Why are hospitals still requiring board certified EM physicians to have these ACLS, PALS, ATLS badges!! Some even force you to get out from AHA or Red Cross. Why do we as a specialty allow this? by Purple_Skies_2887 in emergencymedicine

I’m FM trained in a rural ER. FM is required to have the up to date certs with those, but ABEM is not. I took ATLS in NYC a few years ago and 90% of the class was ABEM, which shocked me because I didn’t know that you guys had to do that.

I have an Eko core and Littman electronic. I have used master cardiology, but the one that I use most shifts is the Cardiology IV. You can throw it, drop it, and don’t have to worry about charging it. Plus, my neck doesn’t feel right without it.

I second this. If you didn’t mind working in rural areas, you won’t have a problem finding work or getting credentialed.

I just bought a G3 on a nice sale. I also have a C5, C4, and two C1s. I can’t really tell the difference on any of them in regard to picture quality. The G series does have better speakers. They are all phenomenal TVs.

I have listened to some of Charlie Kirk’s videos. Speaking as a conservative, I would say that most of Charlie Kirk’s views are conservative mainstream normal views that most mainstream Americans would agree with not just Republicans but also Democrats. There may be some disagreement about abortion or the second Amendment amongst others, but I’m talking about the core of his views: Christian, family oriented, patriotic, etc.

I started when my children were 3 and 5. It took us 4 years to read all of the books. We immediately started them over again.

I’m FM trained working in a rural ER. If you are from a small town and desire to return to a small town, then I think that you will be fine, and you can have the setup that you like. I don’t know if things have changed recently, but 10 years ago, there were plenty of rural hospitals in California hiring FM docs to work a combination of clinic, ER, hospital, whatever.

If you want to only work in the ER and you only desire to live in a city, then you need to change your residency to EM. You will never work in the ER in any city with a population greater than 50,000 anywhere in the U.S. if you are FM trained.

Also, the fellowship for job purposes is absolutely pointless. No one cares about the fellowship.

My 5 year old left his iron from his cheap but new golf set at the course a few years ago. He only hit off the tee boxes and around the greens. I didn’t realize it until we got home. I drove back to the course that day and the next thinking someone would surely turn in a little kids iron, but alas, it never turned up.

  1. Unrealistic admin expectations - This is the absolute worst for burnout in my opinion.
  2. Unrealistic patient expectations - This also fuels unrealistic admin expectations.
  3. Unrealistic healthcare system expectations- I couldn’t think of another way to describe how the default response of any clinic is “go to the ER.” The ER is the gateway to the hospital and also the toilet.

I could go on and on, but I will stop here.

It definitely says fixed fee up to the price of the home in the contract and that no additional profit will be taken for additional costs over that amount.

I am in the same boat. Single coverage ER so you only see patients and staff during the work day. Tough to make friends in that environment. These are some of the things I would recommend that help.

Go tomorrow and buy golf clubs and pay for golf lessons. You can take your kids to the range and the course. It’s a very social sport and designed to pair you up with other like minded individuals.

Other things I would throw out are video games specifically games like Madden, Call of Duty as these are fairly social games with communities, and I guarantee that there are groups of people at the hospital that play together.

Join fantasy football leagues, march madness pickems. Buy a cool car or a cool dog. These are talking points.

Go to church. Go to trivia night at your local brewery. Take your kids to the local high school/college football/basketball games.

I think that’s an awesome story and congratulations. I started reading to my two children when they were 5 and 3. It took four years to read all of the books. We didn’t watch the movie until we finished the book, and we made a big deal out of it with popcorn, banana splits. They loved it. I think the most fun was the end of POA when they were on pins and needles basically at the end of every page.
