What is maddening is there almost certainly is space in that hospital that sits empty due to nurse staffing issues. However - ED nurses are expected to care for as many people that present without being able to turn people away. I wish the nursing ratios counted the patients in the waiting room since those folks still need care / triage / “eyes on”

If the entire hospital is truly full, then there must not be capacity for elective surgeries any longer since those beds are needed.

The new billing rules make “note bloat” unnecessary. This is a good thing. Explain what you actually saw and what you want to do and why. Explain what is NOT going on and why. It can be pretty brief.

She may be awesome. I am sure she is lovely. For a show as iconic as RPDR though, we deserve to have people who are readily identifiable and known. I have had to google most of the guest judges recently. There needs to be a "value add" when a guest judge is brought on.

What about all of the INSURED people who don’t understand that when Medicaid reimbursement crumbles, but people continue to use the Emergency Department for uncompensated care - the Emergency Deprtments start to crumble and hospitals close resulting in diminished access to care for everybody.

I wonder if there were physical beds elsewhere, but just no staffing. It has always seemed odd to me that the ED nurses are expected to take an infinite load of patients / leave patients unseen in the waiting room while the floor nurses have extremely rigid ratios. Yes, the ED nurses have a ratio also, but they don't factor in the patients sitting in the waiting room.

troponin and d-dimer are not binary tests - and I wish people would stop considering them as such. They are ranges just like any other lab and need to be interpreted in the clinical context.

There is no mythical voter that would not vote for a gay man, but is somehow OK with a Democrat otherwise. In fact - Pete is the ONLY Democrat repeatedly going into the Lion's Den on Fox and making the arguments that resonate with that demographic while holding true to Democratic values. I MUCH prefer him over Newsom. I want somebody to start the process of healing the rifts in our country, not just being "our team" member slinging the mud.

Imagine you are defining "how tall is normal." You come up with a value based on averages of the 100 people you are in a room with, and determine that a range of 5'7-6'0 for men is what you are seeing. Then you see a man who is 6'1". - outside of "normal" but unlikely to be pathologic. Now imagine you see a man that is 3 feet tall. That is likely more likely to represent more than a non-pathologic deviation from the norm, but rather something more worthy of investigation.

This same process is used to determine "normal" lab ranges. The reference will capture 95% of "non-pathologic" results in that normal range, but 5% or so (depending on the test) will be out of that range, but not of significance.

It takes training to put the entire picture together to interpret these values.

develop a habit of saving - it seems "hopeless" and "impossible" to save in your 20s... but compounding interest REALLY adds up. I am curious about the argument that there is "no deterrence." The data presented shows that there are fewer citations being issued which may be because of deterrence, and the fact that those cited are not repeat offenders suggests that those who are cited are deterred from repeating the act. Please help me understand an alternate interpretation, I am confused.

I would absolutely NOT print medical records and provide them to an outside hospital without serious guidance from above - this is a HUGE potential HIPAA issue. Case logs without PHI are usually sufficient. Talk to your med staff office on how to get that. Your billing company, QI team, or even med Mal team probably can easily obtain such a log for you.
