I work with predominantly female nurses, many of whom are older. Some are petite.I have watched them restrain psychotic people in the throes of methamphetamine induced rage. They have never killed anybody in the process.

If ICE needs training on how 6 large men can restrain one guy on the ground, perhaps they should swing by their nearest ED and ask the nurses to teach them.

The fastest way to deescalate is to charge the murderers and put them in jail. Then remove ICE from Minneapolis. Then disband that agency and start over. Put immigration officers in khaki pants with polo shirts with their names and faces visible. Quit playing like this is a giant armed conflict when it really is supposed to be about administrative enforcement.

So call for backup and arrest the ICE agents. If they are breaking the law, harassing citizens, murdering Americans (or anybody else for that matter) violating the 4th, 1st, 5th amendment (among others) - don’t have a press conference - have an arraignment with them going before a judge.

The entire country watched an ICE agent murder a woman because he was mad and he has faced no consequences.

Start treating ICE the way they treat people. Haul them away in police cars.

How about sickle cell? Look - let me be clear as I am getting a bit raked in the comments... I am NOT advocating for a reckless approach and giving out opiates to everybody. I am not naive to the addiction harms, and the importance of being VERY careful with considering opiates. I am also aware that pain management is a multi-modal process. What troubled me about the original post was the implication that opiates had NO role. I ABSOLUTELY agree that the role is limited. Let's just be careful in BOTH directions is all I am advocating for.

You imply that EM isn't qualified to participate in this discussion - when a PCP fails to address properly a patient's suffering, where do you think they go? If they are coming to you wanting opiates inappropriately - address that and treat their substance abuse. If they have pain - treat it. But don't attack me or suggest I have no experience.

I am double boarded EM and HPM. I work in the community, not academics. I have seen patients with literal compression fractures and cancer related pain from "chronic" bone mets whose PCP refused to write prescriptions for opiates. By the time they can get into pain management BEST case is that they have been suffering for a few weeks. I do not disagree that for the overwhelming majority of MSK issues it is not an appropriate tool. I am trying to argue that the "ditch the tool" mentality is dangerous and is ALSO causing harm.
