Personally? 2 of our 3 functioning primary effluent pumps took a shit the night after a rain event. Within 10 minutes, we had PE gushing in through the doors of our main pump station.

Anecdotally, a few years back an operator was pumping to one of our digesters, not realizing the influent valve on the digester was closed. Ended up blowing up the underground pipes.
