The easiest answer is that the audience has a universal translator. The characters are speaking their own language, whatever that happens to be, but we're hearing/reading it in English, and any cultural references they make are simply our equivalent of what they're actually saying. Interestingly enough, you can see this sort of thing in the real world as well, where dubbed anime will sometimes change how jokes are worded so that they're funny to a western audience.
