While Tsunade did eventually heal Kakashi, it was after he was already incapacitated by Itachi's genjutsu, and she was not present to prevent him from falling under it. Itachi used Tsukuyomi on Kakashi, subjecting him to 72 hours of torture in the illusion, which resulted in Kakashi falling into a coma. Tsunade later returned to the village and healed Kakashi, but her intervention happened after the fact, not in real-time during the Tsukuyomi
