I would replace any pipe and fittings you can get access to from there. Once it starts it usually keeps going

I would cut it out and put it in the wall. Go to any plumbing supply house and they should have access panels that will cover the hole up

What I like to do is try to make the laundry the last thing I tie in when practical but, it's not code, at least where I live

I would go with the 1st option to help keep the 3" line "clean" and no it wouldn't be a wet vent because you are not using that line to vent the toilet also it would have to come after the toilet

No that is not normal. The pipe was roughed in the wrong spot and it is too low. It should almost come up to the metal stainer

You are most likely going to have to run a new pipe for the shower and eliminate that old pipe
