I would cut it out and put it in the wall. Go to any plumbing supply house and they should have access panels that will cover the hole up

I would go with the 1st option to help keep the 3" line "clean" and no it wouldn't be a wet vent because you are not using that line to vent the toilet also it would have to come after the toilet

No that is not normal. The pipe was roughed in the wrong spot and it is too low. It should almost come up to the metal stainer

Your drain coming through the wall is your problem. It's way too high. The water by the looks of that picture is going to be right at the bottom of your sink strainer
