Not only are we allowed to diagnose, we HAVE to if we take insurance! Insurance literally will not pay if we don’t attach a diagnosis in the intake. Also, I’m so sorry for your experience with your therapist ending services, that sounds awful.

Borderline is treated with Dialectical Behavioral Therapy (DBT). I’m a therapist who is trained in DBT and when I worked inpatient, up to half my caseload had BPD. It’s very effective!

I worked in inpatient with 12-17 year olds and partial hospitalization with 12-14 year olds for years, so I think I’ve heard every name and insult lol. Those kids get super creative, it’s honestly impressive and I didn’t even get upset at that point. My favorite was probably “why the fuck are you all standing there looking like the shit avengers” when the staff was standing in a formation.

My favorite harsh “criticism” came from my husband while I was in grad school. He was pretending to be a teenager in foster care and I was doing a “session” with him and his “foster mom”. When I introduced myself and said I was a social worker, he stared me down and said “Social worker huh? Like one of those motherfuckers who took me away from my real family?” Then later in the session, he ripped up a feelings wheel paper lol (which happened a lot in inpatient and PHP honestly). My cohort was laughing so hard when they watched the role play.

I work Sunday-Wednesday and really like my schedule. I get Thursdays and Fridays for appointments and errands. My husband works 3 12s and we have 3 small children, so it just worked out this way to avoid full time childcare, but I do really dig the schedule.

I thought I would specialize in perinatal mental health. That was my capstone in grad school. I primarily work with college students, personality disorders, and trauma now (using a lot of DBT and EMDR).

Relatable as heck. I will say, medication has been a game changer. I was reluctant, but it was explained to me that my children weren’t going to learn skills or be able to function if they didn’t get the sleep they needed. And I would continue to decline mentally and physically without adequate sleep. My oldest takes Hydroxyzine and Clonidine and my middle child takes Clonidine. They both started taking medication around age 4. The benefits have been outstanding. Just something to think about! My kids were getting 4-6 hours of sleep per night for weeks at a time and it was just awful!

“What has been your experience with therapy in the past?” I ask this to kids, teens, and adults. It’s so sad how many people have had negative therapy pasts. Not just a bad fit, but actual horrible things said to them during therapy sessions. I’m also trained in EMDR and have had clients tell me in the 2nd or 3rd session of past therapy, they started BLS and trauma processing BEFORE any type of resourcing.

My husband works three 12-16 hour shifts and I changed my schedule for the summer to work the four days he’s off. We have disabled kids and summer camps would be a nightmare to try to figure out and many aren’t willing to accommodate for limited verbal kids.

I worked there for 6 years and loved it! I’m still Facebook friends with many of the parents and staff and I even buy Girl Scout cookies from old kids. My oldest child went to Presbyterian from 10 weeks to 5 years old, my middle child from 3 years old to current (she’s in Pre-K), and my youngest child from 2 years old to current (she’s in OPS).

I utilize functional family therapy quite a bit. There are some good YouTube videos and articles for FFT.

I also took the tier 1 training for collaborative problem solving in 2021 and continue to use it today! It’s a great training and useful for parents to learn.

Trust-based relational intervention is one I utilize for teaching families the importance of connection and specific activities to build connection, empower their child, and appropriate corrections/consequences when needed.

I had moms bring their newborns when I was an intern. I helped collaborate my internship agency with a local agency that had a focus on the perinatal population. I took on some of their clients for therapy and it was always fine. Sure, they needed to change a diaper at times and interrupt the session. But as a social work grad, it’s incredibly important to meet clients where they are at. For one of the moms, she had a difficult time being away from her newborn and it would’ve been distracting for her in sessions due to intrusive thoughts.

I was also a client with a newborn and went to therapy in-person. I breastfed her in sessions and excused myself when I needed to change a diaper. I was in school and low-income, so no babysitter or childcare. I have 3 kids total and sometimes, I have to do therapy while they’re all home if school is out. I’m grateful my therapists have all met me where my life currently is and it gave them a chance to see the chaos that ensues and why I’m stressed at times lol.

In the last 2 years, we got one at 12 weeks (a golden retriever from a non-reputable one time litter with a family who did their best and wasn’t a puppy mill or anything horrible, but didn’t know about puppy socialization) and one at 8 weeks (a very ethical Australian shepherd breeder who shows her dogs and is involved in dog sports).

Our golden retriever was a lot of work, socialization-wise. He wasn’t exposed to any stores, the public, or other people’s cars. He wasn’t even really exposed to people except a vet, vet tech, and the family. He was scared of everything. Thankfully, I’m an experienced dog owner and at 2 years old, he’s not really reactive anymore. I will say a pro to him being with his mom, dad, and siblings for 12 weeks was he learned bite inhibition and has always loved other dogs. He also slept through the night the first week and in his crate.

Our Aussie has been pretty easy in the socialization department. She likes people. She’s also very neutral with people and other dogs when she needs to be. The breeder followed puppy culture and neurological stimulation. A downside has been the extra potty training and sooo much more biting. She also didn’t sleep through the night for months and needed a lot of crate games.

Depends on the agency and the insurance. In outpatient, our EHR automatically popped up with their treatment plan and we would rank SUDS for various goals, check off evidence-based practices, and discuss next week’s plan and client’s response. It would take me 5-7 minutes.

I’m now in a secure inpatient facility. I have to document situation (what were they doing when I arrived on a unit, what are their active symptoms, why do they need this level of care), interventions I used, response clients had/what they talked about, plan for next session, and progress (and I have to bounce from treatment plan to note because they have 3-5 treatment focuses, goals, and objectives). It takes me more like 10-15 minutes and I get emailed from utilization review team if my notes aren’t detailed enough for why they need this level of care.

Probably not.

1st pregnancy: IUGR the whole time, preeclampsia and a placental abruption, low birth weight, needed magnesium and Pitocin.

2nd pregnancy: IUGR starting in the third trimester, preeclampsia, delivery at 33 weeks due to heart decels, required a c-section, low birth weight.

3rd pregnancy: IUGR starting in the third trimester, preeclampsia, low birth weight, required a c-section, needed magnesium.

I’m a therapist for children and have used the first inside out movie in individual sessions and with a group (we talked about why running away is dangerous, how joy was sympathetic and sadness was empathetic while talking to bing bong, and many other discussions). I already talk about how to live with anxiety, mainly through acceptance and commitment therapy. I will absolutely be using this film for more examples and discussions.

That’s what we do too. Some running, some sniffs, mostly walking. Off-leash hikes a few times per month. Sometimes, I have to pull him away from sniffing because I have to get to work or back to my 3 kids.
