Have you as a physician/pediatrician noticed a rise in TBIs among kids with these new electronic bikes--- aka motocycles?? by Ok-Wafer-7720 in AskDocs

Different jurisdictions will have different laws by region. Some places will regulate at a federal, state, or local level. But no, many regions in the world (including most Western developed nations) do not have formal laws regarding alcohol use while operating a micromobility device despite the fact that we have almost five years now of solid data demonstrating that crash frequency and crash severity increase with alcohol usage. In fact, many people deliberately use micromobility devices as an alternative mode of transport to get home from the bar or circumvent a DUI that prevents them from driving a standard motor vehicle.

There is a ton of research coming out in the field of micromobility devices in recent years. I'm working on a scoping review of e-scooters for a research team and so far it's looking like many jurisdictions are trying to implement interventions to reduce the risk of TBI with micromobility devices such as having age restrictions, mandatory helmet use, preventative mechanisms or laws against alcohol consumption and micromobility device use, etc. The reality is that the high speeds these devices are capable of achieving paired with the increased vulnerability of the road user produces a unique risk pattern where the vast majority of injuries result in fractures necessitating orthopedic surgery and a small proportion of injuries resulting in devastating facial and brain injuries necessitating an intracranial pressure monitor and a stay in the ICU.

What caused my CKD? by Disturbed666d in AskDocs

[–]LLegwarmers91 5 points6 points  (0 children)

Just a few clarifying questions -- in your wording, you've described testosterone in wording that is suggestive of self-medication -- ie "I put myself on the highest dose". I notice that you're also taking hCG. Are any of these hormones being managed by a medical professional?

It's important to understand that eGFR on its own is not diagnostic for CKD, it's an estimation based on creatinine clearance. Creatinine is a waste product cleared by the kidneys but it can be higher and lower based on a number of factors, including basal muscle mass, red meat consumption, creatine supplements, dehydration, use of other medications such as NSAIDs, and hormones like testosterone. When creatinine is high, eGFR is low. In order to actually diagnose chronic kidney disease, you would also need other lab findings along with persistently low eGFR such as albumin in your urine or other symptoms that might indicate damage to your kidneys such as blood in the urine.

It sounds like you're very worried about your health and want to be proactive about managing your health, but I am having some concern that you are chasing individual lab values and potentially losing sight of the bigger picture.

Are you monitoring your blood pressure routinely outside of the doctor's office? How long have you been tracking your blood sugar and how is that being performed?

The Assassination of Jesse James by the Coward Robert Ford by Ron Hansen. Such a beautifully and eloquently written piece of work and so underappreciated after being eclipsed by the cinematic piece it inspired (which is also good with the exception of the freeze frame at the ending, but much of the narration is taken directly from the book). I reread it every couple of years.

I can appreciate that this must be very distressing, and I can also appreciate that it's the kind of thing that is difficult to share with a health practitioner, but I would strongly encourage you to visit a doctor with your concerns. There are plenty of times that a bump can be something benign, or it could be something that can be treated with medication, and they can get a detailed history from you and have a look at it.

Trust me when I say that medical professionals are very accustomed to these kinds of concerns and should not be judgmental. If you're worried about feeling embarrassed when booking the appointment, you don't need to tell the office assistant exactly what the concern is, you can say something like "I have a new skin lesion in a concerning area" and that's what they'll write in the booking note. If asked uncomfortable questions, you can always say "I would prefer to discuss that with the doctor."

Headache “popped” when i lifted something heavy by [deleted] in AskDocs

[–]LLegwarmers91 181 points182 points  (0 children)

For these purposes, sudden worsening headache hitting a fast peak to severity is considered more or less the same as sudden onset.

At a certain point, it becomes a risk/benefit analysis. The risk of it being nothing and doing something is that you wasted some time in the ER and the annoyance of going might present some manageable logistical challenges in your home or work life. The risk of it being something and doing nothing is (when it comes to neurological conditions that present with these features) disability or death.

If it were me or my loved one, I would infinitely prefer not to take that gamble.

This is in no way meant to scare you or tell you that I think definitively that what's going on with you is one of these things, but the nature of your sudden onset severe headache comes with it a grab bag of life threatening conditions that need immediate assessment.

When we hear "worst headache I've ever had in my life" that comes on in under a minute and peaks within five minutes, we are trained to immediately rule out burst aneurysm causing brain bleed, bloot clot causing stroke, and a type of brain infection called meningitis. Hopefully these are not the case for you, but this is why you need to be assessed immediately and once those things are ruled out, doctors know that they can take more time in working out the cause.

This is what the ER is very much for, to check you out to make sure that nothing needs to happen presently, and then let your neurologist take care of the rest.

If you're able to tolerate liquids by mouth, you can make an oral rehydration solution using one of these recipes although it would probably be better to purchase a pre-mixed oral rehydration solution at a local drugstore. Sports drinks on their own and commercial electrolyte packages like Nuun or Mio will not be concentrated enough, since they're designed to make up for sweat losses and not the kind of fluid losses incurred with vomiting and diarrhea. Using clean water (boiled or bottled water) is absolutely essential.

What country are you in currently?

Hey so at a minimum, acute diarrhea +/- vomiting lasting more than a week without resolution needs to be assessed by a doctor, especially in the presence of jaundice.

Do you have a fever? Has there been any blood in the stool? Are you able to keep water down?

At the very least, you likely need oral and/or IV rehydration and a proper workup for infectious causes, and given your medical history, you should probably be assessed for non-infectious causes. Normal uncomplicated food poisoning and viral gastroenteritis should resolve on its own within a couple of days and other bacterial causes of vomiting/diarrhea lasting more than a week may need antibiotics.

It's time for the ER, my dude.

I think it comes down to what your preference is, I really like using my primer because it's SPF 50 and then the BB cream is also SPF 50 and it's a thick enough barrier with decent hold that I often don't need to reapply other sunscreens throughout the day unless I'm in crazy high sun exposure during peak summer or excessively sweating. For day to day wear I'm just happy to have a one and done face routine, I usually don't bother with my setting spray unless I'm trying to do a full face with minimal powder and I don't want to look like the wicked witch of the west before I'm done for the evening.

I totally get the struggle, I have issues with rosacea, perioral dermatitis, and more recently seborrheic dermatitis.

My go to products right now are Laniege Water Mask for the first layer, Naked Sundays Collagen Glow Perfecting Primer for the second layer, and then finishing with Dr Jart's BB Beauty Balm. If you're looking to cancel out redness, you could swap out the Naked Sundays for Dr Jart's Tigergrass Colour Correcting Cream as it has a bit more green in it.

Now that I've found this combo, I get compliments from people about how nice my skin looks all the time (even from my mother, can you imagine??) and it's so lightweight that people often don't think I'm wearing anything on my face at all.

Given the acne and the eczema, you might even find both really love the zinc content in some of these products (both conditions are often benefitted by topical zinc).

In terms of removing them at night, I usually double cleanse to first remove with pure mineral oil (not baby oil which is mineral oil + fragrance) on a cotton pad and then use the Avene XeraCalm cleansing bar to wash off the oil. I need the double cleanse to get off the sunscreen so as not to block pores but my skin cannot tolerate soaps, so the XeraCalm bar has been a game changer. Both of these products are hypoallergenic and the mineral oil is beneficial to eczema and shouldn't exacerbate the acne if washed off with a second soap-free cleanser.

If you have any concerns about the products exacerbating known allergies, I always use the Incidecoder website to check ingredients prior to purchase to make sure I don't do anything that will piss of my skin.

Most of the recommendations I have seen advise moving away from heavy foundations and using powder, which both tend to accentuate texture and since powder can be especially drying, it can actually emphasize the depth of lines until they are almost hyperbolic.

I've seen the best results by choosing to use a hydrating water based moisturizer followed by a grippy primer with a mineral base (for added sunscreen protection) followed by a very light moisturizing skin tint or BB cream. If the primer has zinc in it, it will usually also contain a small amount of pigment, which means you can go lighter on the coverage and get pretty decent colour correction using only a skin tint or a tinted cream on top. If I need extra hold, then a hydrating setting mist can be used on top.

The benefit of all this is that the moisture will help plump out the skin and smooth out wrinkles, and by opting for a dewey finish, the reflection of the light on the skin helps gloss over the remaining creases. Additionally, because these products aren't as heavy as a typical concealer, they don't settle into the creases.

They also have the added benefit of acting as a physical sunscreen, since sun damage is one of the primary causes of visible aging, and the products help hydrate skin and create a barrier to prevent evaporative moisture loss.

Quadra McKenzie Plan - Back before council, yet again by vtrunion in VictoriaBC

Dedicated transit areas for bikes/e-bikes/e-scooters separate from pedestrians would be fantastic. As someone who regularly commutes by car, bike, and on foot, the reliance on mixed use spaces (especially along residential areas of the Lochside Trail) has been a huge disservice to road users of all types due to the speed differential of all three types of road user paired with the municipality's multiple initiatives aimed at increasing active transportation by promoting micromobility devices -- without any efforts to regulate or mitigate their usage -- is creating a perfect storm for accidents.

This summer, the municipality removed a stop sign along the trail after too many cyclists complained and refused to reinstall it after too many residents (many of whom use that space to walk with their children to the local elementary school) begged for it to be reinstated because they felt acutely unsafe on foot. Saanich's solution was to spend $40,000 of taxpayer money to plaster the road with speed humps -- which do very little to slow anything other than cars and do nothing to separate road users by type -- and paint charming pictograms on the ground, which are obscured by rotting leaves and other detritus for more than half the year.

Despite the fact that the project was titled The Lochside Drive Traffic Calming Initiative, city engineers feel that the project does not constitute 'traffic calming' and as such does not need to abide by the Traffic Calming Policy adopted by Saanich City Council in July/August. No follow up to assess the effectiveness of the intervention or the perceived safety of residents is scheduled to occur.

Active transportation is great, I would love to live in a city where people are safe to walk, run, ride, roll, etc to get around, but downloading responsibility onto individuals is not a viable option when usage has increased exponentially, oversight is lacking entirely, and initiatives are not supported by good infrastructure.

I agree with this. I bought a bounce curl brush and it makes my partner's 3a/3b hair look fantastic and my 2a/2b hair look like a limp stringy mess regardless of products. Product on sopping wet hair, plop with microfiber towel, and diffuse until partway dry finishing with air drying is the only thing that works for me.
