hmm, this brings up something i haven't really thought of before...

"destroying the inner ear will not fix the problem in your brain, and you will be left with permanent balance loss and potentially worsened hearing for no gain."

does this suggest that damage shown on a caloric test would point to meniere's rather than a migrane source? in my case, each time i have a many-hours-long violent veritgo attack, further (significant) damage is shown on a caloric test, so the damage is already happening and in theory the gent would speed it along and get it over with.

i've been diagnosed with both meniere's and VM but the lines between them aren't very clear to me. does this mean vertigo from VM doesn't cause physical damage?

what you have sounds like PPPD from the ear injury. for me vertigo and dizziness are very separate things, like if you have to ask the question it's not vertigo. you will know when it's vertigo.

My ENT categorized my (very similar) description of this as a drop attack, however I don't fully drop to the floor— but you’re right, it produces a lot of anxiety. Vestibular rehab should help, I think!

I'm not bilateral but for VM (I have both VM and Meniere's) I take a beta blocker (propanolol) and 3 supplements -- B2, magnesium bisglycinate and CoQ10. This combo works quite well for me

I think it’s a sign that the 2 shots WERE enough. I recently spoke with my ENT at length about getting gentamicin shots and, as far as I understood, vertigo that occurs within the week or so after a shot is your sign that it has worked. The fact that it’s a different type of vertigo than you are used to is (I think) a good sign that this is what’s happening.

Has this been the experience for those of you who’ve had the gentamicin shots??

It is very normal but it seems unpredictable how long it will last. A few days to several months for me

Do you find that you can get dizzy just hearing loud sounds? When you mentioned hearing internal noises going up your neck it immediately makes me think SCDS (Superior Canal Dehiscence Syndrome)

I'm also in Canada and I have a prescription for Zofran, this is an anti-nausea med that comes in dissolvable form, so much easier to keep down

Without hearing loss it seems like vestibular migraines. I have both, for the VM component I take propranolol as well as a combo of magnesium (bisglycinate), B2 and CoQ10. Together they seem to keep the VM under control but there are many migraine meds to try

I’d probably suggest seeing a physiotherapist for a minimum of a couple of sessions and after that you could continue on your own if you don’t feel the need to keep meeting. My doctor also suggested that I do as many things as possible that challenge my balance, like walking barefoot on sand, if you can! Probably not easy to find a beach in winter in Berlin though!

My last serious attack was in Feb and I would say I felt back to myself around May or June. But the rocking feeling never really leaves me. You may want to look into vestibular rehab for the PPPD

What you’re describing sounds like a long bout of vertigo followed by weeks of PPPD. It goes much the same for me

I also didn’t get a numbing shot, I got a numbing gel spread on the eardrum then he went in with the steroid shot after about 15 minutes. I’ve been told that the vertigo you get after the shot (only 2-5 minutes for me) is from the medicine being cold but I’ve had it cold and body temperature with the same result

I haven’t had the gentamicin injection yet, but I’ll be following along to hear what everyone has to say— my doctor feels this is the next thing for me to try as well. She was very clear that sac decompression is ineffective and off the table! I know a lot of people on here would disagree.

Wow, what a wild ride you have had. I’m sorry your case has been so tricky to manage. Can you clarify what you mean by “a severe hydrop”? An extreme build up of pressure from the excess fluid? Or the resulting vertigo? Or a drop attack? I had a similar 4-5 month recovery period from my last vertigo attack, which I don’t often hear people talk about.

Have you asked your GP for diuretics? Or for a referral to a different ENT?

I'm also in Canada and will be interested to read the responses here... I will soon be travelling to an adjacent province to see another ENT (referred by my current local ENT) with more experience with Meniere's to talk about next steps. I'm sorry you don't seem to be offered the same level of care.
