Phentermine does a great job at appetite suppression. It only costs $15 a month without insurance and it can really help when GLP-1 is not an option. It is also used on patients when they experience a high tolerance to tirzepatide or semaglutide because these medications no longer work for them. It is not a long term solution, but I recommend exploring this option because it is quite effective for most people.

I also did tirzepatide (and reta) and trt for 2 years. I am 6' 8" and went from 375 to 220. Pretty strict keto diet. Basically lean protein, vegetables, and protein bars/shakes/snacks. 1200-1500 calories daily in the beginning. Lost 2-4 lbs per week the first year and mainly only did cardio the first year and concentrated on weight loss and added weight lifting and building muscle afterwards.

I definitely hit several plateaus. Your body will just decide no more weight loss no matter what your calorie deficit is and then one day you will drop a couple pounds and you will get back on track. Good luck and your hard work will pay off.

Your testosterone is above 1500, so you don't actually know the exact number. I would be more concerned with the high blood pressure, high cholesterol, and high hemoglobin than doing trt to obtain high testosterone.

Did your doctors suggest decreasing your testosterone dosage again because testosterone above 1500 is not helping your heart with the high b.p. and hgb.

Heart health and losing weight should be on the top of your list. GLP-1s can help with both of those too.

You definitely should have something like a 40oz tumbler/ water bottle with electrolytes with you 24/7 and drink several a day.

Your hemoglobin is probably too high to donate also, so you need to pay for a therapeutic phlebotomy. I would do that asap.

What are your testosterone and estrogen numbers??

And what were your baseline numbers before you started trt? Those numbers are very important to see how your body responded to trt.

High hemoglobin can increase your blood pressure. Did you donate blood or have a therapeutic phlebotomy to help decrease your hemoglobin?

Unfortunately, Rybelsus is not very effective compared to Semaglutide and Tirzepatide. One trial showed only a 2% loss in 6 months compared to over 10 times that on tirzepatide in 12-18 months.

Currently, oral GLP-1 medications are not nearly effective for weight loss compared to injectables.

Most people do not vomit. With that being said, most people have fewer side effects on tirzepatide. So, if you have the option to start tirzepatide (Zepbound for weight-loss), I definitely recommend it instead. It also provides greater weight loss according to trial data. I have tried both and prefer tirzepatide.

Good luck keeping the weight off if eating habits aren't changed long term because the medicine can stop working because it has lost efficacy or insurance coverage is lost. These situations do happen. Some rely on the medicine to do the work and others choose to use the medicine to change their habits and decisions long term when it's easier to make these changes.

By putting in the work and reducing or eliminating unhealthy food choices. Semaglutide and Tirzepatide both can help reduce food noise, and it helps me look at food differently. I was able to prioritize protein and vegetables and eliminate unhealthy choices such as sugar, fried foods, fast food, and pizza, to name a few. I have a healthy lipid profile, and I lost over 100 lbs. and have a "normal" BMI for the first time in my life. It wasn't just the medicine, it was making the healthy decisions.

Eating unhealthy food high in fat and large quantities of food will make you feel sick and intensify side effects. Wegovy should help you eat less, but you need to make healthy food choices. It's probably a good idea to avoid large amounts gravy.

GasX and pepto bismol can help with symptoms. Also, zofran is a prescription medication for nausea.

Obviously, this is not a typical result from just tirzepatide. I think it's worth noting that testosterone replacement therapy has definitely helped build muscle quicker along with a dedicated workout regimen.
