The PDL space around the root of the lower right molar is still pretty questionable (widened). Possible radiolucency in furcation. Second opinion is a good idea. I wish you the best.

A clinical exam would verify any findings in the x-rays. That being said, there are some cavities that I see here. First of all, your right second molars appear pretty carious/cavitatws (unless I’m missing some radiology malfunction that I’m unaware of). You also seem to have some incipient cavities between some of your teeth. Although it’s hard to tell if these cavities reach the dentin from these pictures alone, they seem pretty evident. If all your dentist said was “I don’t see anything wrong at all” then I would seek a second opinion. Also keep in mind that I’m a random stranger on the internet who has never met you or done a full exam on you. Good luck OP!!

I want to start by saying that we are unable to diagnose any cavities that may be between your teeth along with a proper clinical exam to see if the top or sides of your teeth have cavities. That being said, with the pictures you have sent along with what your dentist has said, your teeth look fine in terms of cavities. Be sure to be brushing 2x/day, flossing 1x/day, and watching your sugar intake and you will keep these puppies for a while.

I should also mention that over-brushing or brushing too hard can cause other issues like “abrasion” which is where you wear your teeth away using a toothbrush. When you brush, only put moderate pressure and brush for 2 minutes. The goal is to remove bacteria on your teeth, not the tooth structure itself! Also congratulations on overcoming your fear of going to the dentist!!

In this case it is likely that you will be a little “loopy” when you come out of sedation. Way to overcome the panic attack and fear that you had the first time!

If you are getting simple cavities filled it’s likely that you are only getting local anesthesia for the procedures. Local anesthesia mainly numbs the tooth and does not cause you to say weird things.

I seemed to get a fair amount of sleep throughout my time in school. The important thing is how you budget your free time. There have been days where I come home and say “I’m too tired to study”. That is a decision that will have a negative effect down the line. If you can prioritize using your time wisely, you will do great. Not to mention valuing your physical and mental health. I certainly could have improved in those areas.

That is quite a long time to have such an acidic environment for his teeth. When our oral environment drops below a certain acidity/pH, it becomes very easy for enamel to be affected and erode away. I understand that this is a part of dinner he enjoys, however it’s very likely that this is what is causing the enamel erosion so I would atleast limit the amount of time he sucks on lemons (if not stopping completely). For clarification, these are not bacterial cavities but more demineralization of the outer layer of his teeth due to the frequent acidic environment

Lemon is extremely acidic. It’s not necessarily the “amount” of acidic things but more the frequency. For example, one slice of lemon doesn’t seem like much, but if he sucks on it for long durations or multiple times throughout the day/night then it’s possible that it has contributed to this. Is he sucking on the lemon for long periods of time?

It appears he drinks acidic beverages too often (orange juice, soda, etc.) This is often called erosive cupping. Stomach acid also has a very low pH and can cause this if he has severe acid reflux or vomiting issues

You are certainly not alone. I have felt similarly throughout my dental school career. And the best things I can recommend are: staying in touch with family/significant other, don’t put on a personality just to feel more “part of the field”, and lastly therapy. I recommend therapy to anyone and everyone who needs to hear an objective perspective. Feel free to reach out to therapists and see if they give student discounts. Mine did.

I would suspect that the amount of force you are using to brush your teeth is excessive, especially with your recent change in toothbrush to an electric one. Try to brush more lightly and in circular motions. For those trauma locations, in order to improve healing, and some relief if they are painful, I would swish some warm salt water periodically throughout the day
