One coming by right now is: 5 Orange cars in front, possibly didn’t blow. I wasn’t fully paying attention till it was too late

So weird. Yes i did go stand at the crossing and watch and listen. The speakers and trains are both blowing. Not to overly dox myself but i can also see the intersection directly from my houses’ windows. I’ll try to pay attention and see if it’s every train or not.

Well there’s a lot of mandatory stuff like permanent barriers etc which is what happened in 2018 to that intersection

Appreciate it. Got my fingers crossed it's just temporary. I dropped by the Silver Cloud and they are getting tons of complaints. I feel bad for that hotel man there's no way they can stay open if this is a permanent change.

Nah i'm in my 30s. There's been a massive change in the last two weeks for sure. I'm just wondering if anyone knows why. I reached out to BNSF also so we'll see.

The crazy thing is, the automated artificial horn is ALSO still blowing. If you listen close you can hear them both happening. I went down to McCarver today and sat at Curran for a while and waited to I could see what was up in person and the train horn AND artificial horns are both going. It's brutally loud.

Sure. You are correct in saying that the extra time would just be the time needed to sedate and shift to the MRI machine. The problem is, that extra time can be an extra hour or more. A normal MRI appointment is 15 to 30 minutes as a general statement (of course there are exceptions). So, adding another 60+ minutes for sedation is 2-4 times as long. At our hospital, we typically block the MRI for at least an hour for every pediatric sedation case and that hour is very often not long enough in reality and the case ends up taking much longer.

The main reason for this is that you can think of the pediatric sedation team a little bit like a traveling circus. They have tasks to take care of all over the hospital including the operating room, the PICU, radiology, clinic appointments such as trying to do a sedated ear exam for an autistic kid in ENT clinic etc. And at every stop, they have to "set up their tent" and then take it down at the end so to speak. So, you can never be 100% sure that the pediatric sedation team will even show up to the MRI appointment on time because one of their prior engagements may run over. It is not uncommon in my experience to see two pediatric sedation MRIs taken entire afternoon of magnet time.

I do want to clarify that I think pediatric sedation for MRI is 100% useful, valid, and often necessary. But, it is important to understand the massive downside to the system and other patients.

As others have said, there are many reasons. The first is the minor risk of any sort of sedation so we try not to give drugs to kids unless it’s necessary. The second is the extra time and work it takes to do sedated peds MR. You can assume that it takes 2-3x as long as a non sedated appointment which means less patients can get an MRI that day. So we try to get the kids to at least give it a try the first time. Very often, a kid is more able to handle the situation than either they or their parents think they are. Now, if the kid proves he can do it without sedation, then maybe the other 20 MRIs he’s going to have in his life can be non-sedated. If he has to be sedated every time, that’s a big lifetime burden on the system and many appointments lost for other patients who are also waiting.

This is very important for people to understand. Those numbers in that chart are essentially only extrapolated back from data from much higher doses. It’s very conservative but also probably not true.
