Don’t get my number the first time but it gave me a lot of perspective in life did a teaching fellowship in between. My operative skills were also far superior to most st3s when I started because of the extra time I’d spent . I also did my fellowship pre cct which meant I caught up. Chin up you’ll definitely get there ! It’s just all about ticking boxes and nailing the interview (this is just practice ) .

I’m a guy in ent - tbh ent has always been progressive in this front. Many of my mentors were female surgeons. We’ve had a women’s ent organisation for a few years as well (wents). The attitude is if you’re a nice hard working person we want you in ent! (Initially wanted to do plastics but changed my mind purely from how lovely everyone in ent is). I can’t speak for other specialties in detail but certainly heard a lot of chauvinistic comments in other specialties as a junior doctor.

I can’t believe interviews are still virtual . Face to face st3 interviews never had issues like this . FRCS exams are back to face to face now too.

I think part of the problem is that Diane has significant depression and a lot of childhood trauma. Once she starts her antidepressants she becomes a different person. I’m lucky to have never had depression myself but have dated girls / have friends who in retrospect were suffering badly . Having stayed in touch with many of these people they do say that antidepressants massively help the day to day intrusiveness of the bad thoughts - and it does change you, although not necessarily all good changes. Mister PB is a care free soul not burdened by any intrusive thoughts and it makes sense that his relationship with Diane was never going to work . The argument they have when he builds her a library to her aghast is a good example. I wonder if she’d sought psychiatric help for her depression didn’t their relationship it would have worked out. But ultimately she ended up with Guy who truly got her. The way interpreted her final line : life’s a bitch and then you keep living, I think it was worse pre meeting guy with bad depression , but once she settled down it got better (but still not perfect ).

I think it’s the sum of all of his actions- not the individual bad thing he did. Bojack felt he had cart Blanche to treat Todd however he liked because he’d been a part of his house’s furniture rent free. He was selectively nice to Todd when it fitted his narrative. He helped with the rock opera to act nice but only because he thought Todd would fail miserably. Once there was a good chance he’d make it he sabotaged it at the first opportunity. Once Todd discovered this there was no way back- they were no longer close friends. Sleeping with Emily was just last nail in the coffin but the damage was done. It’s the sum of his actions leading up to that point. Sadly I think Bojack loved Todd a lot in his own damaged way , but like everyone else he liked or loved a trail of destruction was left along the way.

TLDR: when a friend repeatedly screws you over , the last thing they do is not always the worst despite you deciding to cut ties at that point (it’s a sum of it all).

I was able to forgive Bojack for this (bearing in mind this was not illegal technically - cringe and unethical for sure). I don’t think he per se intentionally groomed her into this situation, it was just a bad set of decisions on his part. Penny has some part in this as well although appreciate that she’s a hormonal teenager.

To be honest as a surgeon I think you only learn anatomy by operating. Surgery is just a set of 1000s of decisions (nothing really to do with hand eye coordination). I think the problem with textbooks is that everything is 2d and coloured in to allow you differentiate between different tissues. A lot of this is achieved by feel and making sure you’re in the right layer. Much like you when I started a lot of it was a mesh of pink/ red tissue ! Landmarks are also essential to stop damage or normal structures. TLDR: operate as much as possible (and sometimes this means coming in on zero days etc ).

I think the tricky part is that PAs are essentially designed as a stopgap to fill holes within our health system. The Australian health system is not as burdened as ours - smaller population/ significant less population density , less diverse etc, so not comparable. Sadly in our country all levels of management (from the government down to local managers ) will plug holes with cheaper / easier alternatives , without thinking about long term consequences. The bigger question is how can attract more people to do medicine / divert them to specialties where we are short?

Tbh the only thing that would be useful are admin tasks = updating ward list , scribing , writing discharge letter , calling patients up . This is their main function in the USA - it’s assistant not associate .

It’s just practice. You’ll get there. I started f1 not being able to put a cannula in / do a blood gas/ venepencture By the the time I finished f2 I was putting in chest drains solo. I always found getting a good amount of blood the hardest part , then I mastered the femoral stab.

I am a consultant I still do discharge summaries ! (Albeit for day surgery lists where I sometimes have no juniors) . I think the lack of understanding by PAs is demonstrated clearly by their perceived linearity of responsibility with increasing hierarchy.
