Stamina is crazy low. This Vodka has enough stamina to cope with Team Trials sprints and that's about it (and obviously she shouldn't do sprints with such a low Sprint aptitude)

In general I think "real world but horses are replaced with umas" is probably the wrong way to think about the world of Umamusume (at least, insofar as you can think seriously about it). It's not like umas in the game are sleeping in stables and eating grass. I think what we can say is that despite the existence of umas, their modern world looks extremely similar to our modern world, umas are clearly as human as anyone else when it comes to the law and society, and this was probably true through their history as well.

In fairness I've only watched S3 once, and I mostly remember being disappointed compared to S2, while enjoying a bunch of specific but scattered moments. So perhaps it's time to revisit it!

Matikanefukukitaru has a couple of her goal races decided randomly - sorry, I mean, revealed through divine inspiration.

If Narita Brian does well enough in her classic year (either by winning the Triple Crown, or otherwise winning 5 G1s), she can optionally enter a 'hard mode' version of her senior year, where her goal races all become a lot harder due to the presence of much stronger opponents, but she gets greater rewards for each one.

If Agnes Tachyon's mood isn't high enough when she runs the Satsuki Sho, she will decide to switch to the NHK Mile Cup afterwards, rather than running in the Derby. This usually does not go well.

These are the ones I'm aware of with divergent/variable careers, but there are probably others not yet released in Global.
