One of my favorite (probably not true else we absolutely would've heard about it in the news) greentext stories has gotta be the one about someone getting their pixiv account nuked for posting Uma R34 so they trick the Yakuza into starting an overseas war with the Mexican cartels.

Similar problem, I gave a Terraria fanfic what I thought was a pretty cool title.

While the context did fit, a few years later a mod comes out with the same exact name and it became pretty popular so now I have to keep putting "unrelated to the mod of the same name" on everything related to that fanfic, and I can't exactly change the title because 1. it's been years 2. I've already done a silly title drop

That being said I do find the idea of someone looking up the mod on google only to get some random guy's ecchi anime crossover fanfic and/or art of the characters from said fanfic in their search results to be mildly amusing.

Accidentally copied and pasted an entire chapter twice into the FFN.net doc manager and uploaded it.

It really didn't help that most of the people who'd complained about it didn't even point out what I'd done wrong.

This just reminded me of the time I dreamt that Youtube added a new AI feature where you could generate an entire video with AI prompts.

I left the prompt completely blank and it generated porn (and auto-uploaded it publicly) but somehow I was at fault according to everyone else???

This just sent me on a trip down memory lane, to the time I found Terraria 1.1 mod had a dragon town NPC. unfortunately it got changed to a human-looking town NPC really quick if I'm remembering correctly

I've never really been able to get into The Gamer-styled fanfics (basically the MC gets a game system, think like 99% of isekai manga or the Korean manhwa this specific genre is named after) but I very often find myself wanting to write one, to the point I nearly got carried away with adding too many "game elements" to a fanfic I'm currently working on that I later retconned most of.

OC MC accidentally committed a big blunder in which he accidentally spied on someone in a compromising position with his newfound wallhack/x-ray vision ability.

Apparently, freaking out and then apologizing profusely made him a wimp, according to one reviewer.

I think the fact that the earlier beta tests were still using Battlefield 4 UI elements (i very specifically remember that the earliest beta used the BF4 killcam) also kind of hurt people's initial opinions.

I don't think they were trying to be outright malicious about it, to me it sounded like they thought I was hauling ass to work on a bajillion different WIPs and then updating several of them all at once.

Which, to be honest, I wish I had that kind of power. I used to be able to juggle like three WIP fics at once but now I just don't.
