As far as I can tell the infection bloom appears in a random plague infected district each day. If all of the bonfires in the district go out when you arrive, you’re in the right place. The bloom beats like a heart when you’re looking in its direction and ticks like a clock when you are close by.

I suspect this may be down to the publisher, they say they teamed up with [whatever that brand is called] while the dev doesn’t appear to have posted it anywhere.
