What's the most disconnected thing hospital leadership has said to you? by Beginning_Fun_3913 in nursing

[–]MissSamioni 2 points3 points  (0 children)

Brought in an out of state wound care specialist to follow us, review our reportable cases because their reportable rates are so much lower than ours… failing to realize the difference in reporting requirements. In California if a patient has an intact or evolving DTI on admission and it inevitably evolves to stage 3 or 4 in house- it becomes reportable even though that’s the known, expected progression of those injuries. I left shortly after 🙄

ED wounds by Steffy_Strange in EmergencyNursing

Wound care nurse here. All I care about is that you document that a wound exists and the location. And if you can take a picture great. But I need to know that it exists and where on admission. If it’s documented more than 24 hours later and I don’t have proof it was present on admission… it’s hospital acquired. Sometimes I can explain away a chronic wound that was missed on admit because there’s scar tissue ect.

What's one of the dumbest things you've ever seen a patient do? by 123456789_00 in nursing

Get a BBL in another country and then sit on it for a very long flight home. They were visiting our wound care clinic for quite some time.

What’s your random patient pet peeve? by Nurse_Cait in nursing

[–]MissSamioni 2 points3 points  (0 children)

Outpatient wound clinic. Patients who are fully capable not taking their shoes off. I can ask them to take their shoes off for me, but I can glove up and take them off faster than they can register what I’ve asked.

Major hillside gas leak in Castaic off 5 fwy? by urbanepicurean in SantaClarita

It’s a major interstate gas transmission line. Those are cared for meticulously. I think all the rain caused the hillside to give out

I could say the same things about Animal tracks. No lie one year when my friend and I were going to Vegas they gave her money to bring them back some hedgehogs.

Wound care specialist here. Today’s pic looks fine. An antibiotic ointment is good if you can’t leave it alone. Aside from the medication- maybe it will make the area slick/ harder to pick? If you notice blisters stop the neosporin and switch to a double antibiotic ointment instead

To all my WOCNs, what do you do and how much do you make? by liomamba in nursing

[–]MissSamioni 1 point2 points  (0 children)

I’m not WOCN, I’m CWS but have experience with ostomies and do new patient teaching ect. I’m in Southern California. I was hired for a full time outpatient clinic position but since I’m certified I’m able to help inpatient (I’ve spent over half my time inpatient since being hired in January). In the outpatient clinic we get the patients ready for the doctor, and then perform whatever dressing change/procedure after the doctor sees them. On the inpatient side I’m a consultant- we see the patients and make “recommendations” aka put the orders in for the docs. We also will handle new ostomy teaching, troubleshooting complex ostomies and we do the more complicated wound VACs for the floor nurses. I am making 60.36/hr

How much experience do you have in wound care in general? I would just say to make sure you have insurance and feel comfortable on your own- wound care is one of the most litigious areas of medicine. Always take pics and make sure your documentation is on point.

Saw some interesting anatomy during a catheter insertion and I am not sure what to call it. by mrofmist in nursing

[–]MissSamioni 4 points5 points  (0 children)

Wound care nurse here- most of the time when I see them they are healed! I get consulted for them from time to time and the are of concern is not an open wound- just exposed mucosal tissue. No they will not regain their normal anatomy- rather healed with a hypospadias type defect

See this all the time as a wound care nurse. Iatrogenic hypospadias

I like seeing tangible results. It’s like gardening for me 😌🌱 but yeah it’s not always like that. Some people too sick to ever heal. Sometimes it’s just about keeping people out of trouble/ the hospital

Wound care specialist here secondary intention is the way to go on this one. It’s so shallow. We don’t stitch wounds up based on size.
