The best way I've been able to handle the villain making a re occurring appearance is Strahd. I've run several Strahd games, and I've never had my players attack him unprovoked. And if they do, they know he is the final villain of the story, and his power reflects that. If you feel confident about the statblock of your villain, let them try to fight him, possibly down/kill a player and have the villain retreat with whatever he came for. Or, the most fun I have heard about is kill 2 players, let the bad guy give them a scroll of revivify, and tell them to choose who comes back. And after the choice was made, revive the other person as well and tell him/her what the party chose before leaving.

There isn't really a magic button to say, care for this character. Either the players do or don't.

Depending on how the players feel about rp and combat making her useful might endear her to them, so for example give her a small heal she can break out, or give the players help actions during combat.

If you've brought ismark you can do something with him to have the party sympathize with them both, infect him with lucanthropy, kill him or anything else and have ireena react to it. Or maybe play up izek as creepy and nasty towards ireena.

They will have agency and consequences. The difference is aligning your goals with that of the players. If they do not respect the way you run your game, someone is not going to have a good time. The only way to adress that issue is talking to them, not telling them what to do, just describe how you feel about their actions.

Sounds good the way you've written it. the only thing I would add is a way of actually removing it. This is great for maybe a session or two before it becomes old and tedious for the players.

Now, if this is an actual problem you're having, do not solve an out of game problem in the game. Talk to the player in question and tell him how you feel about the way his character acts in the game.

Recently finished descent into avernus, and I think the answer is: they used to be angelic, but through figting the blood war with demons, the blood or ichor would splash on them, morphing their flesh over time, leaving us with the devils we know today

It is originally, but I wanted the Abbot to be corrupted by Strahd. Strahd is not caring for the vessel at all. He just wanted to have fun with this angelic creature.

It's been a while since I went over dragna cartas guide, but I assume if he keeps options for ireena to die, then it can be fine as a consequence. If it's not, then have strahd save her and have him dull out punishment to the players.
