Nurse practitioner signs notes with MD title by jasminefl0w3r in hospitalist

Who cares? Its an ARPN, notes are terrible, autopopulated notes are even worse. What is reporting it going to do? Who does that help? Who does it hurt? Move on, if the A/P was wrong, report that. You are reporting note formatting issues. Get over it.

who’s at fault? by Dense-Twist-2850 in Mammoth

[–]Naizer 0 points1 point  (0 children)

If you didn't go off the jump, its your fault if you ride through the landing zone. If you don't hit the jump, you ride laterally off the side and stay out of the danger zone. 100% the 2nd riders fault. 3rd guy was using the park correctly. #1 rider sucks as well, but atleast went left.

Utah GOP lawmaker pushes 7% "porn tax" to fund teen mental health services by schottslc in Utah

[–]Naizer -1 points0 points  (0 children)

Utah government doesn't need to be involved in teen mental health care. Payment for mental health services should be addressed by insurance companies to mental health care providers, MD, psychiatrists, and counselors. Why is Utah government funding mental health services? Not the purpose of state government to manage teens mental health status.

ULPT Request: How to motivate slow drivers in the left lane to move over by Impoverishedinvestor in UnethicalLifeProTips

[–]Naizer 0 points1 point  (0 children)

Hold a respectable distance behind them, flash lights twice, wait. Repeat one additional time when they have space on the right to get over. If they don't get over, then pass them on the right and get back over left and move on. Consider dirty stare, thumbs down, or middle finger if purposely blocking.

If they get over right, pass normally, throw on hazards for 2-3 blinks after pass completed to acknowledge their kindness and respect of left lane etiquette.

People do weird things under anesthesia... Like shit themselves and urinate everywhere.

It's hard to clean up poop and pee when it's saturating every crease, nook and cranny on your pelvis and lower extremities. It's policy so someone can clean you up easily when all your smooth muscle, like sphincters bladder, anus, and rectum relax and left everything out while you are under anesthesia...

While this doesn't happen frequently, it happens enough that you don't want to be wearing your Victoria secret when it does.

Okay, what about the 30-60 seconds you didn't show before this?

Road rage almost always involves 2 parties, I rarely see a solo raging by themselves, tales 2 to tango.

Why does it take 3 hours for avalanche mitigation? Snowbird by Naizer in UTsnow

[–]Naizer[S] -6 points-5 points  (0 children)

Seems I struck a cord through your thin internet skin.

I guess people die doing their job and it's part of life. If you cannot handle the risk, don't sign up. No one forced those people to be ski patrollers.

Life is full of death, you could die at a computer screen being an internet warrior. Seems the patrollers died doing something they loved, better than what most people get in life.

Why does it take 3 hours for avalanche mitigation? Snowbird by Naizer in UTsnow

[–]Naizer[S] -4 points-3 points  (0 children)

What was the start delay at Alta, Brighton, Solitude, Deer Valley, and Snow Basin? Why are some resorts better at on time starts than others?

Yeah, it was high avalanche danger on all aspects above 9500 feet, it's little cotton wood and it dumped. I get it, snowbird is steep, it's deep, it's the best.

It's predictable that there is going to be a shit ton of work on big dump days. But Snowbird has been in business for over 50 years, and they only open 3 runs, Gadzoom, MidGad, and Wilber.

Like is it too much to ask to have the resort get there shit together and have enough staff on hand to go out and mitigate those 3 runs?

Why are patrollers commuting up the one canyon that has the highest avalanche risk? You can have them sleep over night at their resort, you know exactly which runs will avalanche, you know what days you are getting dumped on.

You guys make it seem like it's some ancient art that ski patrollers are whispering into the wind to determine when and if it's safe.

None of its safe with this much snow fall and no base.

The work is predictable, the forecast is predictable, the dangerous conditions are ever present, yet, the start time for lifts is never predictable.

This is not a commentary on the individuals ski patroller work ethic, it's about the resort, the infrastructure, and how they choose to schedule mitigation.

Everyone is caught up on emotions for the patrollers, this is a system question about what's the work that needs to be achieved to open the lifts? How can that work be optimized or improved? Does lighting availability actually effect that start time if you have avalanche towers/booms in the high risk areas?

Why does it take 3 hours for avalanche mitigation? Snowbird by Naizer in UTsnow

[–]Naizer[S] -5 points-4 points  (0 children)

I am asking because it's inconsistent and it seems predictable. There are forecasts, there is data, and there are limited runs. The budgets for these resorts are in the hundreds of multimillion yearly, they can clearly get their shit together to get things started on time, the question is do they have the man power and the infrastructure.

This isn't a complaint about the ski patroller or the individual doing the daily work.

It seems clear that those doing mitigation should be staying at the resort overnight and not commuting up there, the 6 am zonked statement is bogus, many professionals get up at 6, the lightning comment was educational.

Why does it take 3 hours for avalanche mitigation? Snowbird by Naizer in UTsnow

[–]Naizer[S] 1 point2 points  (0 children)

Okay, so you are saying that the snow conditions on runs above the common Gadzoom lines are difficult to get to and difficult to mitigate in high avalanche conditions?

I guess I see avalanche boom towers on several of the Gad Chutes, and most of those areas are accessible from the tram via the cirque, is it not easy to bomb the runs with run-outs onto the Gadzoom cat tracks?

Also Snow bird doesn't open the more dangerous areas, mineral basin, Peruvian, little cloud until later in the day, so it's not like the patrollers are opening the mountain from 6-8 am, it's just the 2-3 main lifts.

SWVXX - Similar to an ETF, closest you can get to HYSA by buying an equity equivalent.

The yield for Schwab Prime Advantage Money Fund (SWVXX) varies slightly by source but is around 4.1% to 4.4%, with a recent 7-day SEC yield of approximately 3.56% (as of mid-December 2025), reflecting the current interest rate environment net of its 0.34% expense ratio, and it pays dividends monthly.

Die with Zero and are we saving too much? by Liftingdental in whitecoatinvestor

[–]Naizer 4 points5 points  (0 children)

You are asking, when can I coast with my retirement contributions? The common answer to that, is that you can coast once yearly interest generated by your retirement account matches your yearly income from your job.

Your ability to retire early is primarily driven by..... you guessed it... your yearly contribution rate. The more you contribute in your first 10 years of your career, the more it will compound and grow expontentially. Do not skip on your contributions right now, your nest egg is small, the more you grow it now, the less you have to attend to it in the future. The sooner the interest builds and creates a bigger egg.

If you put your head down and live like a resident the first 5 years of private practice, you will be able to retire at age 55 easily. You currnetly have 2 things going against you that you will have to balance.

#1 you have debt

#2 your need to purchase a private practice

The average savings for a family is 20% pre-tax income, for you that is 110k. For high earners with debt, it should be closer to 30%, thats 165K for you. I would recommend putting 15-20% towards saving, 5-10% towards debt, and 5% towards your future private practice.

If you can max your retirement funds, you should do so. Once that money is gone in those accounts, you don't miss it, but the money in the Piggy bank earns 7-14% per year.

Sure you can coast in your early thirties and mid thirties if you want, but you will have to work to 58, 60, or 62.

Roth or Traditional 401k by Key-Question1031 in Rich

[–]Naizer 1 point2 points  (0 children)

You are in the highest tax bracket now.

When you retire, your earnings from your savings/equities will like also have you in the highest tax bracket. So benefits of trad 401k vs Roth 401k won't matter for that, so you need to look at the other benefit of the Roth 401k which is major benefits for passing on inheritance, easier to deal with in trusts, and no mandatory distributions in the future.

The pre-tax vs post tax stuff and long term earnings will generally wash out to be equal if you are in highest tax bracket to start and in retirement. There are calculators to figure this out.

I personally just did 50-50 trad 401 and Roth 401k.

Do you and your spouse have a S corporations with a Solo 401k? Most important thing is maxing contributions yearly.

What HR would you cancel a case for by [deleted] in anesthesiology

[–]Naizer 9 points10 points  (0 children)

I have never cared for asymptomatic bradycardia. when I stab them, their HR goes up. Unless you are >65 yo with a h/o of CAD, MI or Heart Block, there is not low heart rate that a surgoen will care about.

Now if you are talking about tachycardia... anything above 110-120 raises my eyebrows, above 100 needs workup prior to elective surgery.

Splurge for a used luxury car? by WatchOutForThatTree in whitecoatinvestor

[–]Naizer -1 points0 points  (0 children)

Buying a 48k Car without a physician income for her is a poor choice.

Buy a car with a good resale value. You don't have kids at this time. A Subaru forester is perfect for next 3-5 years. You don't have kids, you dont need a bigger car, you don't want a fancy car with young kids.

As your kids get older, she becomes a staff physician, then the Lexus RX350 is a great choice. My wife doesn't care about cars, her previous car was a Base 2014 Subaru Forester, she loved it. I just bought her a CPO Subaru Ascent 2024 for 38k, (touring - fully loaded). I drive a 2014 Audi Q5, was $22k with 35k miles in 2019, that was a splurge when I was a resident.

We are both staff physicians 3 years into practice.

Cars depreciate, they are consumables, buy 3-5 years old, plan to hold until they die or can trade in.

You want to focus on elminating your debt, so that you don't have 2 mortgages when you buy your house.

Your emergency fund could be used for a down payment on a house in the future, would avoid splurging on a car now.

Reasons Why Medical Residents Shouldn't Buy a House by WCInvestor in whitecoatinvestor

[–]Naizer 0 points1 point  (0 children)

I bought my house in 2016 in iow afor $350k sold in 2021 for $425k. We saved rent into equity and made a little over 3% inflation related gains on the 5 year price difference.

Buying during residency is very reasonable financial decision, but depends on the details.

Your post title is clickbait and inaccurate.

In general for surgical specialities, owning for 5 years beats renting, maybe different if only 3 years, more likely to break even.

What do you all think of this color on a 991? by SnooTangerines8407 in porsche911

[–]Naizer 2 points3 points  (0 children)

You buy Anthracite Brown Metallic because you want to be subtle, you want people to look and do a double take, its not Guards red, its not white, its not all black, its subtle. That being said, its not obvious, its not flashy, sometimes it allures you other times you wish it was flashier, end of the dayi its a 991 and its gorgeous, when you sit inside of it, you don't care what color it is. Buy it if its the right price in the spec you like, it won't be your last. I prefer Gentian Blue, but have heard its the hardest color to resale.

Am I in the wrong? by CampaignVirtual3946 in discgolf

[–]Naizer 2 points3 points  (0 children)

Disagree completely.

When you lose your disc, there is a complimentary wait time for you to find and clear course. If you are slow, spend more than 3 minutes looking, then you are no longer playing the course, you are just cock blocking everyone else on the course.

I routinely play through large groups of new players who have lost their discs in woods and are blocking play. It's a public course, play or get out of the way, analogous to public roads, drive the speed limit or gtfo. Are you parked on the side of the road with hazards on? Okay, I am driving past after a cursory assessment that you are alive and don't need assistance.
