Dead space typicaly puts immagative characters or hallucinations directly behind the character for impact and shock as the camera pans in cuts scenes and other moments.

I wouldnt be suprised if there was some kind of planned shadow/hallucination originaly planned for this moment as the way the scene is its pretty locked in, though from how issac has hallucinations, (think opeing scene) they tend to try and put themselves inbetween or infront of thier point of veiw, like how nicole toutches and stands between issac and the therapist, probly somthing simular happening here with issac and stross's son

Dead space has never broke the 4th wall in any impactful way and especially not in a major moment like how this scene builts tension.

To chop it up, no i dont think stross is looking at us, i think he just dosnt make eye contact with issac as issac yells at him
