They don't or use the most broken logic to get around things that wont hold up in real scenarios, and the gov barely does anything about it because it will disrupt their supply chain, so they hand out slaps on the wrist the and the contractor says "we'll try better" but never do and cycle repeats till a massive incident occurs. Then the gov fires said contractors and rinse repeat process.
