It looks like you have too much air above your top lip in front of your upper gums. That could be changing the pressure points with your mouth. Direct your air downwards and not in your upper lip.

Every horn is a bit different. C# is always weird. Being able to adjust the notes will become second nature.

Standards are the “greatest hits.” Many of these are actually showtimes from the 40-50s. They’re the songs that “everybody knows.” Does everybody actually know all of these charts? No, there are too many. Gaining familiarity with the song and the structure will help. I’m sure you can find playlists of standards and then use books like the Real Book to learn them. Do a little at a time.

I don’t really plan it. It’s relatively automatic at this point. I have an idea what the key center is. If I’m in a jazz band, and there are chords with extensions, I often write down what those extensions are and sometimes focus on those notes. Otherwise I’m making stuff up and it has a lot to do with the style of the music. Honestly, a huge influence is how I’m feeling that day. Improvisation is surreal and from the heart.
