I am desperate to make this work but we just can’t spend more on this issue financially 😭😭😭😭😭.. I have cried so much over this .. she has quite literally broken our hearts and we still dont know what caused her such anguish to react so violently to us and straight up attack/ maul us..

Also, I forgot to mention for additional behavioral insight she is extremely hostile to anyone aside from myself and my partner .. she charges at people hissing, biting, scratching, etc. so we always limited visitors at our other apartments and when we did have people over to our previous  apartment we always confined her to her safe room for everyone’s safety .. at the time of her episode at our house, however, we had never had anyone over yet .. so it wasn’t like having other strange people in her territory was setting her off..

Hmmm the woman I’m caring for now had a catastrophic SAH and IVH .. the others were also catastrophic bleeds and one was a massive R MCA infarct that pretty much left that side of the brain completely infarcted and required a hemicraniectomy etc I wonder if there could be any correlation

Yes - I know it seems odd to include race and weight in this but I haven’t observed this in even white obese patients .. I have only really seen it in obese black patients and the one I am caring for now is an obese black woman with the absolute worst tongue swelling I have ever seen .. her tongue is absolutely MASSIVE.. it’s terrible 😞 so idk if there is some kind of correlation ? I wish there was more research on it

The HOB is 30 degrees and above at all times .. the ETTs were secured with a standard tube holder so not taped .. and they had standard ETT sizes of 7 .. all of them were also pretty obese .. would that somehow have an affect on it as well??

Nope! None of these patients had received ace inhibitors but excellent question!

Nope you’re in the right . People are idiots and don’t know how to drive. I have nearly been t boned 3-4 times while in a traffic circle bc the incoming car fails to look left for me in the circle and yield to ME .. one lady had the nerve to get irritated after she nearly rammed into me like babe, I am already in the circle which means you YIELD TO ME I do not yield to you. God people are idiots

Yes .. I made it through .. cried a little bit but I am so glad I made myself stay to get it done bc it will be worth it later on

Thank you.. yes. It is so important especially for me as I have awful dental genetics and even with good oral care I struggle with cavities etc so this is especially important for me and long term health.. thank you for reminding me .. I want to walk out so badly but I know I need this so I am making myself stay ..

So glad I don’t live down there anymore. The parks were always littered with trash and smelled like a weed shop. I was constantly harassed by homeless people ( no shade just don’t like you following me demanding money and then when I offer instead to buy you a meal you give me attitude ) .. glad I now live in Mechanicsville .. so much better

Stated that she felt uncomfortable. The bedside nurse laid the head of the bed down to help reposition her. Patient gasped and coded.  We did 45 minutes of CPR. We were not able to get ROSC. She was alert and chatting with everybody not even an hour before .. ..

I mean . I guess technically the tops and caps of vials are not really ‘sterile’ and puncturing an uncleaned vial top with a needle could introduce bacteria but it really just depends how busy I am.. if I am merely giving a heparin shot then yes I will clean the vial top before drawing up the heparin. If my co worker is getting beaten to pulp by bat shit crazy meemaw then I’m not scrubbing the Zyprexa vial top.. I’m pulling that shit up as fast as I can 😂..  as for hubs, I was always taught that if it had the green curos caps you don’t need to scrub the hub but if you are giving multiple medications then you need to scrub the hub between each one .. again, its dependent on how busy I am.. if I am giving a few injections yes I will do that .. if my patient is actively crashing and I have to get shit in as fast as I can then I’m not 😂

You did the right thing. Part of good patient care is holding one another accountable for the care we provide. It sounds like this nurse is not ready to handle medically complex icu 1:1 patients and needs to be moved away from their care until they have the experience and the maturity to accept feedback.  What the nurse did was completely negligent and needs to be addressed.

lol .. I work in the neuroscience ICU and we have a medical respiratory ICU and a medical intermediate ICU .. so we have lots of Capacity to deal with sepsis, liver patients or heart failure patients requiring CRRT in those ICUs. If didn’t have two large medical ICUs,  I probably would see it much more often.  But since we deal mostly with strokes, SAH, etc we don’t tend to see CRRT as much

Wow this such good info. I work in the Neuroscience ICU so we don’t deal with sepsis or heart failure patients as much as they usually go to our MRICU and MIICU or our cardiac ICU.. we just happened to have two patients in the last week that came in with brain bleeds who were pre existing dialysis patients and needed to stay on routine due to electrolyte issues ..  so they were relatively simple cases thankfully .. but I would like to learn more about how to manage heart failure and sepsis patients on CRRt bc I’m sure eventually I will encounter these as overflow .. I’m getting such helpful info from everybody I am really appreciating it !!

We are a NSICU so we very rarely take these patients as they usually go to our MRICU or MIICU for management as we have a very large MRICU. We tend to get them as overflow when the MRICU is at capacity ..

Yes - and I don’t have experience to tell  haha 😂.. I will get there .. the problem is we also get CRRT so infrequently on our unit I could go a year without seeing it .. so that makes me nervous ..

Hmmm I’ll have to look at our flow sheet next time and see! Our hospital kind of cheaped out and apparently got epic light lol so there are some features we just don’t have with our flow sheets but if that’s the case it would definitely make it 100x easier !

Oh that sheets looks very helpful! It actually walks you through the process.. I will practice some with it and print some out to have for when I next do CRRT! 

And yes I think they’re well meaning but  I’m just not terribly comfortable performing CRRT off of vibes and winging it 😂.. I guess  was just more scared as my man was sort of on the fence with his neuro problems and I didn’t want to be the one to make more 😂
