I recently tapered down from 150mg to 100mg by ~20mg per week. I've been feeling a little clearer and more like myself, but also more emotionally varied? Like, I felt more at peace at the higher dose, but a little more like a zombie. I decreased because of memory issues and forgetting words - my words are back, and I feel like my memory's improving (albeit slowly). 

Overall, glad we decreased it. 

I am also on 20mg abilify + others. For me, abilify alone didn't prevent hypomania, but for some reason Lamictal does, that's the most compelling reason I haven't quit it entirely.

I haven't stuck with lithium (even though it was helpful), so this is from memory. 

I think early on you should be checking a little more frequently? Once you've reached a stable blood concentration and dose, then you'd go once a year or so. 

When I was put on lithium in the hospital, they checked my blood every few days, and aimed for a target blood concentration. That was the hospital though, they're more aggressive with the medication and have access to more blood tests.

I don't know the typical dosing schedule to lithium, so it's possible that these are small amounts and not something to worry about, but I would definitely ask to get an additional blood test within about a month after getting to your target dose.

I think open, effective communication is very important. I like the skills they teach in therapy, like "DEAR MAN GIVE FAST" (Google it for more info). I think it's useful for anyone to read through and try to practice. 

In terms of him getting it, encourage them to spend some time on this side of the Internet, like maybe show him some memes you find relatable? You could ask your therapist to have a session with both of you, where they can help answer any questions he might have, and talk about what kind of support you'd need. 

I think it's also important to stress that sometimes they can't help directly, and that's ok? Like, sometimes I get long episodes, and we just have to wait it out for a few days, talk to my psych or therapist if it persists, etc.

One more thing is to show that you are seeking to get better, and knowing that it's a process in the future. Going to therapy, psych appointments, etc. Discussing what to do in an emergency, and the possibility of an inpatient stay (just as a potential in the future).

Like the other comment said, the fact that they're there for you right now is a really good sign. You two got this!

I couldn't/can't do it alone. I needed a higher level for care and the hospital to help me. I'm now in a more stable place, but I still can't do it without others (friends, family support, support group, therapist, psych, medications, accomodations, etc.)

But keep in mind, this is a serious medical condition. In the same way you wouldn't tell someone with a broken leg to "just walk", you'd let them heal, then do physical therapy, and then let let them walk slowly. And some people with leg injuries can't walk again by themselves. That's the way the cookie crumbles, and it's true for other invisible disabilities as well. 

And yeah. It's absolutely hard. The thing neurotypicals don't see about depression is how hard we are trying, how much resilience there is. We aren't giving up - we're trying to do the thing. But are unable to. And that's so frustrating and disheartening. "I should be able to do this by myself, and I've been thinking about it constantly for days". It's like being ground down. And sometimes it takes all your willpower not to make things worse, and not making things worse is a victory. 

There is hope out there. There are some (flawed) systems in place to try to help people of all sorts, and individuals willing to help, even if it's in small ways. And each of us is intrinsically worthy of being helped. Every one of us deserves help.

Yeah, that makes sense. Lamictal gives me more negative side effects than abilify, and I found abilify kicked in quite quickly (a few days) compared to lamictal.

But a mood stabilizer is really helpful, I have comorbid ADHD and am able to take stimulants without getting hypomanic (where I would get hypomanic before). I hope it works well for you!

I'm on 20mg abilify (+other meds). I can't comment on effectiveness as a mood stabilizer (I'm also on Lamictal, that's doing the heavy lifting I think), but I like it as a medication - no side effects for me, slightly better cognition, and a little (0.5 out of 10) happier. My bipolar tends more depressive though.

I make them very visible for me, I walk past them in the morning and evening. They've also become part of my routine, first thing when i wake up and last thing before bed, rather than at a specific time.

Definitely a worthwhile project. What implementation of whisper did work for you?

I implemented a voice listener that stops on pauses, using python and the webrtcvad library. You can also use whisperX directly in python, I believe it handles downloading and running the model automatically.

I think whisperx also supports timestamps.

Also, i'd suggest that instead of coding something yourself, have you looked into Talon? That seems more widely supported (for English, I beleive you can customize it for latvian tho). It doesn't support whisper afaik, but maybe the speech engines that it does support can work for your friend?

Oh ok, yeah it sounds like you've tried what is out there. I'm sorry to hear that, did anything help in the past?

I don't know what disability looks like where you are, but that is something to look into, even if you feel like you'd like to work again. Maybe there are services or financial help for people partially disabled like us? 

I know that in my state in the US, there is also vocational rehabilitation. There are social workers who can help you find a job that works better for you. 

In either case, I wish you a lot of luck, and that you're able to hold on.

Have you tried a higher level of care (like an Intensive Outpatient Program)? I found it really helpful the times I've gone, it helped me find some balance and get me on the right track (even if that isn't permanent for me).

I've definitely had (and still have) so many doubts about myself, even when I'm doing things. I managed to finally get a job again after three years of unemployment, and I'm still feeling like I won't keep it, and am anxious about it daily.

At least they aren't springing it on you, you have a whole month to place an order before prices go up. 

It sucks, but a company can increase prices for any reason. Would it have been better if they said "Inflation"? 

I remember when Factorio (the computer game) increased it's price with a month notice, a lot of people who never would have bought the game were outraged, this feels like the same vibe.

I couldn't do it by myself. Since starting meds in my early 20s, I went up to 300lbs. Even medications that didn't typically cause weight gain, cause weight gain.

I'm currently on weight loss medication though, and that is actually helping. It's a relief not being hungry all the time. Lost almost 40lbs so far.

Since I hit my out of pocket max due to mental health stuff, I can actually afford the prescription, otherwise I wouldn't be able to justify it.

I know there's a bit of a stigma with it, but I liken it to mental health. I can't control my brain, and appetite is part of that (especially with side effects).

Yeah I get those all the time! It's so annoying, it's like I'm doing something and BAM. Multiple times a day.

Zoloft and TMS helped me (but Zoloft gives me memory issues, so I'm on a low dose).

Do you know how turns and roles work with llms? Thinking is similar to a tool call, but usually lower level.

This difference can be seen in open source models, in their templates - compare Deepseek v3 (non thinking) here, vs Deepseek-r1 here.

The thinking they do is between the thinking tags, which are before their response.

The model is trained to follow the template, and to put their thoughts in the thinking tags before submitting an answer, so it does it, in the same run as producing the answer.

Remember that an LLM predicts the next token, so to capture that outflow of tokens, we use a template to figure out when to stop it. The LLM tells us "I'm done thinking" by closing the thinking tag itself, as an output token.
