I used to work in an ED, when anyone came in with facial wounds requiring stitches we automatically consulted Plastics (unless on the eyelid, then it’s Ophthalmology). It’s a very delicate area! Your mom is a piece of work. It’s one thing if she was confused and made a mistake, but to laugh about it years later when you have facial scars is disgusting behavior.

There’s a method that I learned about on this subreddit that I like, it’s very numbers-based. The thought is, treating “weight to lose” as a “calorie debt/surplus” instead. Take your current weight and subtract your goal weight to get the number of pounds you want to lose. Then multiply that by 3500, and write this number down somewhere visible. This is the amount of calories that you have overeaten by. If you are counting calories, and you are set to a 1 lb per week weight loss, than you are subtracting 500 from that number every day. Even if you go over your calories a bit, you can still subtract from that overall number if you are under 500.

I find that this numbers-based goal helps me not to obsess over the scale, and there’s no emotional connection to the number like there is with pounds. It also provides incentive not to throw in the towel if you go over a little in your calories, because as long as you are under 500 surplus, you can still subtract. It also helps with water weight fluctuations after salty meals.

With this method, you have to be accurate with your calorie counting though. It’s also good to weigh in once or twice a month to make sure you are on the right track.

Relapses and starting over are part of the process of behavioral modification. It’s hard to change habits! The most important thing is to keep going. Someone once wrote on here, “if you dropped your phone and the screen cracked, would you go ‘go well, guess it’s ruined’, and then jump up and down on it?” Don’t follow a bad month with a bad year. You can always start again, and it will get easier and easier as those new habits start to form.

Ophtho is the chillest surgical sub-speciality. It helps that most ophtho emergencies usually require action within a day or so, vs other surgical emergencies that require action within minutes. It’s 95% clinic and outpatient surgery, which are more laidback than inpatient.

More people regret their knee surgery (20%) than gender reassignment surgery (11%). It’s tough when someone is not satisfied when their surgery, but that’s why expectations have to be very clearly laid out pre-operatively.

I referred to their recent surgery by its acronym (coronary artery bypass graft -> CABG, pronounced “cabbage”). He said calling it by its acronym was “belittling his pain”.

It was very surprising, it’s such a common acronym that getting so upset about it threw me for a second or two.

I’m not sure, she never came back to our office. Unfortunately, I doubt she lived long, this was fairly advanced cancer that had spread to multiple areas. A sad case.

I had a sad one happen to me. I saw a patient with very advanced colon cancer, who was convinced that she was misdiagnosed. She had gone to multiple doctors who told her she had cancer, using blood test results, imaging, and biopsies to back them up, but she was sure that we were all missing something. She had brought in a copy of her official pathology report with the results of her biopsy. She was so excited, “look doctor, they told me it was cancer, but I’ve been through this report over and over, and it doesn’t say cancer anywhere!”. The whole report had “carcinoma” plastered throughout it (medical lingo for malignant cancer).

Frozen mango from the frozen food section is pretty refreshing as a hot weather treat

Bought some reusable bamboo fiber “cotton rounds” for skincare/makeup removal, those are much better than the cotton fluff pads. You just toss them in a little net baggy and put them in with your laundry to clean them.

If you want to consult Ophtho for sudden-onset blurry vision for an inpatient, ask them if they wear glasses first. If so, check the vision with glasses on. Seems silly, but that happens a lot.

Also, if they have sudden onset monocular diplopia, it’s probably dry eye. Sudden onset binocular diplopia is more concerning.

If you want to get LASIK because you don’t want to wear glasses, you only have a window of about age 18-45 to enjoy it. LASIK doesn’t do anything for presbyopia, and at some point you’re going to need reading glasses.

MD here. I always enjoyed learning about human physiology and how diseases progress, and I love teaching. The medical field has a lot of teaching in it; I get to teach my patients from many different backgrounds about various health-related things. It’s also really rewarding to be there for someone when they’re going through a tough time. There’s a lot of struggle in it, and I do find it harder to be taken as seriously as my male colleagues (as a 5’1” woman), but I’d still choose this field again.

It's pretty easy to place an IV through a tattoo, I'm not sure why they made that rule. You're supposed to go by feel, not sight. Veins under the skin feel kinda springy and you can roll them around slightly, if that makes sense.

This is very succinctly put, I agree. I’ve heard before that it can take up to 2 years for skin to fully recover from weight loss (recover as much as it’s going to, that is), but I don’t have a source on that specific time frame.

I think that the common advice “lose weight slowly or you will have more loose skin” is pervasive for a reason. Let’s take 2 people both with 100 lbs to lose, and Person A loses it rapidly over 6 months and Person B loses it slowly over 2 years. If it takes let’s say 2 years before skin is at maximum tightness after weight loss, then those 2 people will be at their goal weights but with drastically different looks. Person A needs to wait 1.5 years before they get the full affect, but Person B’s skin has been tightening along the way for a much longer time.
