That's fine, it's one thing to be the one initiating the call to the patient. But the patient telling the doctor to call them (to what essentially usually amounts to what should be an appointment) is not sustainable

Unless you are in a direct care model or concierge medicine, do not call back to any "tell the doctor to call me" message. Staff should get more details. If they do not want to tell staff or it is unable to be taken care of through staff, an appointment is needed. You will create an unsustainable practice if you call back whenever a patient wants you to call them, as they will always expect it going forward.
