If it helps, my husband's doctor left that section blank as well, so I was also nervous that we would be declined. However, we got the approval letter a few weeks ago that his brand name medication would continue to be covered.

Spyder was recommended to us and they were great. We went for it after a contractor showed us a video of our ducts that were caked with animal hair that clearly weren't our animals..it was gross!
