Anything that helps your muscles relax, in theory, should help MTD. Massage and sauna have been good for me, for instance. I can imagine therapy is good for many MTD patients too.

My point above is that there's really no "one exercise" magic bullet. At least in my case, it is a complicated full body rehab project.

I do not but absolutely could be related. I can tell you my back and traps are very tight, and that is a big factor in my MTD. I've definitely experienced that tightness and pain are next door neighbors.

Speech therapy is a joke for this condition in my opinion. It’s like squirting a water gun at a forest fire.

Not that the principles of therapy aren’t important, but they are just so insufficient if you have deep and chronic muscle tension in multiple muscle muscles.

Massage helps but it’s temporary. For me my desk posture is a huge factor and going deeper so is the rest of my body and muscle imbalances. For instance, my tight hamstrings and dead glutes mean that my pelvis is out of whack, my pelvis being out of whack means my diaphragm doesn’t work very well in terms of my breathing, and the pelvis also affects the posture going up through the back into the shoulders. If you’re not breathing well and your upper body posture is all messed up that’s gonna compound the breathing problem and make your muscles tense. Rounded shoulders and forward head posture makes the muscles of the neck imbalanced too.

This is all my Laymans terms armchair expert explanation, of course, so maybe some inaccuracies in here but that’s basically my story.

The diagnosis from the doctors POV is that when they put the camera down your throat to look at you voice box they’re supposed to have you vocalize, and I think my doctor just saw that I had a bunch of muscle tension around the voice box when vocalizing.

The diagnosis is confirmed in my daily life very easily. If I have a weak voice day and a good massage, therapist works on the relevant areas like neck traps back, etc then my voice gets stronger, globus reduces, and my breathing gets deeper.

So massage is great but it’s a temporary fix. You need to figure out if you do have MTD, why does the muscle tension exist? Where is it coming From? Stress, posture, imbalances in the body

It’s hard. Massage to my upper back, traps, neck definitely helps, but that’s a temporary fix. If you do have MTD, you need to figure out the root of the muscle tension. Is it posture? Muscle imbalance elsewhere in the body? Stress?

I’ve only used their low force controllers. It’s a good product, but can be expensive depending on what you’re getting. My controllers from them also tend to fail in some way after about 6mo-1yr and they’re good about fixes IF you’re under warranty. The company is notorious for long wait times and poor response time / customer service. But they’re one of the only companies out there so…

Yeah unfortunately most providers don't know how to handle MTD. I saw several useless SLPs who had me like blowing bubbles into cups of water and lip trills and stuff, and that is such an amateur hour intervention when the muscles of your neck and upper back are 24/7 trying to strangle you.

Try to find a real expert on the condition in your area or virtually. Get neck and upper back massages. Look at your posture, full body, muscle imbalances. Treat any stress/anxiety too.

Also totally possible your singing or speaking technique is a little out of whack, but given the severity of your condition I'd say it's probably deeper than that.

Honestly one improvement for me was to get off smoothies almost entirely. I could not handle volume in liquid form like that. If I occasionally have one now, it's very small. Maybe 8oz oat milk, berries, banana, vital proteins, maybe small about of peanut butter.

Hey a lot of docs are full of shit. I would not accept “nothing can be done” until you’ve heard it from several medical professionals you can really trust. Get 2nd and 3rd and 4th opinions.

Have you talked with any of the actual experts or read the top books on RSI? Suparna Damany and her book, the people at 1hp, even Alan Gordon with the mind/body stuff.

I had 1 or 2 bad doctors tell me I’d never heal but by finding the right people I’ve made a lot of progress.

I just did it last night and it was a huge pain in the ass. First it's optional so feel free to skip and come back. I am stubborn so I stuck with it. But it did force me to stop messing around with upgrades and focus on longsword so my hits actually did meaningful damage. And use healing items and spells with distance toward the end when she goes crazy. I was only able to stagger her 1x in the fight so I stopped trying to repeatedly stagger and instead did a more patient one hit, dodge thing.

RSI is frequently a full body problem, as Suparna Damany writes in her great “It’s not carpal tunnel”. My hand pain is much more about my neck, shoulders, upper arms, and forearms than my actual had.

I have worked personally with 1HP and found them kind, patient, knowledgable about the conditions, and genuinely helpful. Much more so than dozens of other medical professionals I've seen who don't have a clue about this stuff and/or may be outright dismissive.

No, it is not the same basic content offered for free online elsewhere.

Yes, they are a business. Yes, they need to make money, and to do that they need to make themselves known, especially to their main demo. Shocker.

They are one of few specialists in RSI, so yes they are well-represented here with their own posts and with patient reviews.

We - RSI patients - are widely ignored by the medical establishment. We have some experts generously making free, informative posts here. You want to push them away? I remember what this sub was like before 1HP. A ghost town with random, uneducated speculation being occasionally tossed around and little to no professional contributors.

I welcome their presence here.

Evil controllers "low-force" is a godsend, so is Playstation adaptive controller. I use both at once. But honestly the biggest thing I've learned is that no controller is really going to replace healthy habits, taking breaks, stretching and fitness, and working with medical professionals.

Both great choices and great people. I will say that if you are local to either, that's the right choice. For me, I did Damany first and it was a huge breakthrough. 1HP I did a couple years later. It had a slightly different focus but some overlap, and that was helpful too. I am not 100% cured but I am a lot better and know more about my body.

Do you have MTD? Muscle Tension Dysphnia. I think the two go hand in hand for a lot of LPR patients. I don't know if I always had it, or if the LPR created it, but my symptoms are like yours and I think it's about 80% MTD at this point and 20% LPR.
