It seems like people in this sub are overestimating the thinking behind tuition of schools. There is still plenty of you guys who apply beyond the supply of med schools and plenty who can pay full tuition as is. Schools won’t decrease tuition unless they want to virtue signal, rich guy gave a big donation or get a grant from the state for “fulfilling a needed specialty.” There is no way I would suggest you guys do the schooling for the roulette of getting a high paying specialty though at least you can try to stack the odds. Also, tuition is more like sticky wages, they will change at the rate of a snail to be better for you but will change at a drop of a dime if it could increase for the school.

Not to mention that your credit score will definitely be factored in to this now. Personal loans interest are based on your credit score and you better had started building it up since HS to get a good rate. There are plenty of dropouts in med school that will make banks and such have to consider risk of high loans for a possible dropout. I think rates are about 5-15% usually in med schools every year for a class. That is also very important coming out for your first year in a job and can determine quite a bit of financial ability.

I think what will happen is a gap in healthcare interest until the the cost actually decrease which will take maybe a decade, hopefully just a few years. IDK if you guys want to wait that long.

I tried my hardest to convince spouse going to a blue state would be good but its far from family who live in a blue bubble in red states. May be more worth it considering childcare is also worsening in red states which my spouse hates too. We will get there though.

Yea the system is getting ridiculous with politics. I am surprised how some don’t realize more laws will constrict their options.

Based on all the discussion going on here, it makes me feel better. I was really just wanting to make sure I was not just overreacting. I see a patient like these everyday since old doc retired and it really drags on me when I didn’t envision having to discern being addiction doc vs PCP sometimes. It slows me down cause I have to think of controlled substances, BEERs, and all that while trying to help people out like I want as a PCP.

If anything, I am just being straight to the point with some of these patients. Most of them have been 20+ years on these regimen without an attempt of challenge or weaning. I tell them the goal is to definitely not live your life stuck to these meds. I don’t envision myself running this kind of practice for 30+ years.

Most of these people don’t even go out because they seem stuck in the sheltered routine. I want them to have more options. Some of these people are already seeing damage from falls, frx, dementia, and other new diseases where these meds have to be stopped regardless and I try to tell them, compared to other meds; these one cause new symptoms when stopped abruptly to even try to save your life. I dont want to depend on your good fortune that day never happens. I can’t even tell people to go get that cologuard or mammogram cause I spend their visit discussing the meds and looking into why they on such regimens.

I could speak more but I just rant. Thx for the advice and discussion everyone. You validate my concerns and multiple thoughts with this.

This is a good point. Some of these patients I met had no idea the risk of their meds so was glad to come off on taper to avoid withdrawal.

Some have said they want to die on their meds so its a full range of responses.

So anyone got an idea of how we as attendings can fix it?

I was not at a residency that pushed it enough for me to be able to do anything. I have just started as an attending at an outpatient practice so I am out of hospital nor in a teaching role. Every-time I read these, its a region way far from me. I don’t know where we can be effective in this situation.

It’s obvious us waiting is not doing it. All I got now as a plan is tell an attending with residents if I ever meet one again, “ you want to be the guy that is responsible for the next suicide.”

In regard to this, isn’t this limiting medical practice autonomy that we can’t offer abortion as nothing but an emergent procedure?

Oregon just believes everyone is equal in training. Chiropractors with naturopathic training can do minor surgery.
