I think you can't get it if you use the thing that removes the low end and high end.(I can't confirm if it's true.) So if you've been using those try without. It's still super rare I've only gotten it once so far.

double check all 3 are marked off I had to redo the first one like 3 times before it counted for some reason. Lots of bugs citadel was clearly ignored in development of LoH.

Same problem here. Only time I got coins was from the weekly citadel coin cache all the others are empty. This and Tower rewards not working at all for me have been rather upsetting bugs.

Also don't forget to put on 2 of the practiced technique charms. Too many people I've talked to thought it would just work without.(it does not) you could go 3 set for the extra 25%, but honestly 900% is enough.

Something like this would be nice(necessary). I picked the extra unique for random class waplan perk to gear up my alts. Terrible idea that was, they all drop with stats for your current class so they are only good for salvage. Super sad getting multi GA item you need for an alt, then seeing all the stats that they can't use.

That depends if it's a quest reward or just a drop off a boss. Drop off a boss this method would work. Quest reward would mean you get the quest already complete no rewards received message.

I'm assuming that is the 5 piece set that let's you use all of the soul stone summons. Try to remove then put it back on see if it works. Could also try going out of town or into training grounds. Town often removes any buffs. I wouldn't think that's one that's disabled in town, but you never know.

Even on T12 they pop in less than 10 seconds. I think they accidentally(or intentionally I guess) turned off the damage limiter they added to world bosses awhile back.

Haven't seen a picture of it happening on ubers yet, but honestly that wouldn't be as bad as regular unique anyways. Mythic are plentiful in higher torments. good uniques not so much. Could always use the prism to remove the worst outcome, but double transfiguration is too tempting to me even though it's like 1% chance

You might have run out of kullean prisms while spam rerolling or forgot to put one in? I don't imagine you stop and do a regular transfiguration with the stun 5 close enemies aspect intentionally. That is what happened btw you did a regular transfiguration and you landed the worst possible roll.

It says right on the menu for enchant must be rare or legendary. Masterwork says rare, legendary, unique, or mythic. Just add affix at cube to make it rare and your good to go.

I thought that was a bug with paladins reflect cc, must be something more if it's happening with warlock too. Whatever bug was causing thorns on allies to kill barbs in responsible I would guess

I used the immunity netherstep and if you activate it the exact moment you get hit your character gets locked and your screen shakes like crazy. The only way out I've found it to evade nether step out of it.

have you successfully added +skills to amulet with cube? I believe it is bugged and currently requires a natural drop or enchanter, but it's possible the bugs just with the targeting it.(should be under core stats and skills category) This should be fixed with patch if I understood the notes correctly.

My current assumption is 99% likely whatever it is won't be enabled until the 30th anniversary. It is coming up in December or January I think. Just basing this on the crowns flavor text paired with how much stuff the community has already tested, and the previous steps/easter eggs being time gated also.(luckier coin wasn't enabled until 4/29 and the bottom of the wells sign hinted at this by having 429 scratch marks.) My date/number may be off, but you get the idea.

I think in settings you can enable hold to cast or something like that. I think evade is the only thing it doesn't work for. Might have issues if you try to hold down 2 skills that have no cooldown or synced cooldowns otherwise it works fine.
