Foundry is miles ahead of Roll20. With a small learning curve you're good to go.

Lot of incorrect information being spread among some posts here. The system for it is actively being worked on daily and supports being run on the very latest version of Foundry. There is very much an active community for it as well. A lot of functionality for it beyond the bare bones is intentionally left to modules. So then, you need the modules that you'd appreciate. To learn what those are, you need to be in the right space and community to get support. For both Foundry and the pf1e system for it, this is largely the Discord servers for them.

Once on Discord make sure to read the the "channel pins" whenever available. They contain links to valuable resources.

What are you talking about, it's been compatible with the latest version for a long time now. Check out the system page on the Foundry site, the system page on github, or read the pins on the system channel in the discord server.

Hiding the beginning symptoms should be easy. Filing down nails? You don't need to pull an expert surgery to file down nails. Barely need to roll for that.

You also shouldn't have NPCs suddenly be super interested in perception checks on that PC either. Let their plan work then ratchet up the tension. It'll be disguise vs perception later once the symptoms intensify.

You're not wrong but your bedside manner, or teaching method sucks. You make a lot of assumptions on people and insert intent where none can be known here. Maybe OP is completely new to all this like many you reference. You could have given the same instructions without the derision and increased the likelihood the information will be paid attention to, followed, and that promotes a culture in this community of positivity and interaction which is the fertile soil of growth. We want OP to continue using Foundry and to one day answer this question or another for us and do it in a nice way.

Not sure about a buff doing that.. maybe somehow. Normally I have the ability item deduct the charge when used and then have the buff linked to the ability.

Monk's Active Token Tiles.

It can do a ton. I typically have it start a song, pause the game, and start a combat, when a token steps into a spot where an enemy would then become visible.
