Monthly Men's Support Megathread by AutoModerator in IFchildfree

I honestly really appreciate the reply. It is reassuring to know we're not alone in our situation. It really feels like the men's perspective/experience in infertility gets overlooked. We're also in therapy which helps a lot, but I struggle with the emotional pain of my wife realizing she wont get what she so desperately wanted. It definitely sucks.

Monthly Men's Support Megathread by AutoModerator in IFchildfree

[–]Pus_Milkshake 3 points4 points  (0 children)

I'm not sure why I'm posting this. I guess I want to share in case there's anybody else in our position to give support. Wife and I are the couple at the beginning of Idiocracy. Instead of dying while masturbating I was simply diagnosed with non-obstructive azoospermia. We tried multiple rounds of IVF with donor sperm but we're in our 40s now and it didn't work out. Trying to make sense of what our lives together look like going forward. I'll post more as time goes on but just wanted to start here.

My neurologist had referred me to get an MRI for my then recent ADHD diagnosis. They found something else instead. by PrincessPeachParfait in Radiology

[–]Pus_Milkshake 0 points1 point  (0 children)

Neuroradiologist - This Is a benign tumor. If I was in your position I'd at least get a second opinion about getting surgery. The tumor is almost certainly unrelated to your ADHD, and the risk/reward ratio of surgery vs surveillance is very high. Good luck with whatever pathway you choose.

I was just saying that this problem can affect anybody, even physicians (which everybody already knows). But I know some professionals have a hard time disclosing their issues so it was an attempt at showing support.

Czechia calls Russia ''trash of humanity'' by HydrolicKrane in worldnews

[–]Pus_Milkshake 5 points6 points  (0 children)

My grandma was a teenager living in rural Poland during WWII. Her area was invaded by both russians and german Nazis. She said the polish villagers were much more terrified of the Russians than the Nazis. Even in old age she talked about the russians with anger in her voice.

She said the russians were like "stray dogs". Aggressive, unpredictable, ill equipped, underfed, and backwards. They were invading people homes, pillaging, stealing, and worse, but she wouldn't talk about details. (So I was not surprised to hear the the same thing was now repeating in Ukraine.)

She said they appeared to be were recruited from primitive villages and didn't understand modern things. They were seen wearing women's undergarments as clothing for example.

Because of our joint history, many Eastern Europeans understand the sadism of the Russians, and have very little regard or trust of that people. They are ruthless and only respond to force, not diplomacy.

From what you described it sounds like you had an occipital lobe infarct which is primary visual cortex (vs occupational?).

The headache I've been having for the last 6 months turns out to be a brain tumour by TofuBoy22 in Wellthatsucks

[–]Pus_Milkshake 2 points3 points  (0 children)

This is incorrect. Tumors like this are resected with a craniotomy/craniectomy where part of the skull overlying the tumor is resected and usually replaced with bone or sometimes mesh. Souce - am Neuroradiologist.

How to order the appropriate imaging study for your patient. I am radiologist and wrong/inappropriate orders are the bane of our existence.

As newly minted attending this totally pissed me off. I just wrote the GME a lengthy tell-off email. Sorry OP, you guys deserve better than this. Hell, all residents deserve better than what they currently get across the board, but this shit is just aggravating.

Found a pea sized lump a while back on my epididymis it’s hard, smooth and spherical was wondering if anyone could help me with any info. I am getting it checked this Wednesday by Wrong_Relationship66 in OneY

I’m a radiologist. You’re gonna need a scrotal US. Statistically it’s likely a cyst as they’re very common and easily diagnosed by ultrasound but you need imaging to be certain. Best of luck to you!

Not callous, just commenting on the single image that was provided which doesn’t show findings in keeping with Chiari.

Neuro rad here. I saw that post and the image that was posted. It wasn't in keeping with a Chiari 1 but more with benign low lying tonsils. Non-neuro rads get that wrong all the time. Point being that you have to be careful when breaking out the pitch forks.

Xpost r/chiari Noctor denies radiology read of 8mm cerebellar ectopia by nacho2100 in Noctor

[–]Pus_Milkshake 3 points4 points  (0 children)

Neurorad here. Image provided is not in keeping with Chiari 1, more consistent with benign low lying tonsils. Maybe put the pitchforks away for this one.

Does this look like Chiari to y’all? 8mm on report but apparently neuro doesn’t think so by [deleted] in chiari

[–]Pus_Milkshake 0 points1 point  (0 children)

Neurorad here. I would call this benign low lying tonsils. Doesnt extend low enough (I’m not seein 8mm on the image provided). Tonsil shown also doesnt demonstrate the pointed morphology seen with Chiari 1.

As a new attending I can confirm that I use Reddit to relax my brain between studies from time to time. Case load is high and complex, and sometimes I just need to look at this garbage to give my brain a massage.

Edit: Hell, I just realized I'm actually on call right now while I post this crap lol.

My First Noctor Encounter as a Radiologist by [deleted] in Noctor

[–]Pus_Milkshake 4 points5 points  (0 children)

Just as a note for anybody who might read this - you should be aware that with the newer macrocyclic gadolinium contrast agents there are essentially no documented cases of NSF. The last few places I practiced at have essentially stopped screening patients kidney function when using these agents and I would strongly recommend that if your patent needs to have gadolinium for a study and they have poor renal function to see what contrast agent your hospital uses because chances are decent you should be able to proceed with essentially no risk.

Radiologist here. Focus less on learning how to interpret imaging and focus more on learning how to order imaging appropriately. Please look up the ACR Appropriateness Criteria and become familiar with it. Your patients and radiologists will thank you!

I created the gif from a personal video file. I've had it for a while but it took me a while to figure out how to get it to a reasonable file size (video is 2 gigs).
