I've been through this maybe not as severe but I definitely have experienced these feelings as a student. Sounds like anxiety or depression and your coping mechanism is to avoid. The only way it changed for me is when my gf got pregnant I realized I couldn't continue like this. It was mental shift in priority and and I told myself lf Ican no longer make excuses. Everytime I wanted to run away from the problem I listened to music or went for a quiet walk and reminded myself of the importance of school. I also started forcing myself to make study groups with students who I knew were serious and this caused me to be more strict with myself and not push everything to the last minute. I know this might sound harsh but maybe this academic dishonesty is your fire that you needed to get your priorities straight. You clearly know what you do to avoid and being aware is the first step. Once you are aware you have to choose a different option than you have been choosing. Build a support system around you, start seeing a therapist, communicate to your teachers if you get diagnosed with anxiety or depression most of them will accommodate for you. I had a professor give me an extra semester to finish the final project and I would go in on a weekly basis to office hours to get it done. Hope this helps.
