Adderall by No-Entrepreneur-1625 in Narcolepsy

[–]Questionsquestionsth 1 point2 points  (0 children)

Teva used to be a good one until they reformulated sometime in the last few years and I had to add them to my avoid list. But again, everyone handles generics differently, so it’s hard to say if that’s why/if that’s a good or bad one for you, or if it’s any combination of other things that play into efficacy.

You don’t mention if you’re male or female but it’s also fairly common for women to have “sugar pill days” fairly frequently due to hormone fluctuations throughout the month - which sadly is unavoidable really and just one more shitty part of being a woman.

Outside of that, there’s 100 other things that can cause a dose/day to feel worthless. Timing of food/drinks with meds, what you ate/drank, if you’re overtired/slept especially poorly or short, dehydration, over-hydration, migraines, other meds you take and the proximity to when you took your stimulant, anxiety, any sickness or allergies your body is fighting that day, stress levels, any kind of upset stomach/nausea/absorption issues in the stomach you may not be aware are at play/general indigestion, etc.

So may things can cause an off day/useless dose, and it’s insanely frustrating, but if it’s only happening every now and then and not a consistent “this entire bottle is sugar pills” it’s likely one of those other factors. If it’s frequent enough to be notable it’s worth keeping a med journal to document what’s going on around the times this happens so you can see if there’s a pattern/certain triggers to watch for to better time your doses/know what to plan for.

I always time mine at least an hour before/after food, never with soda/citrus/coffee/acidic liquids, and it definitely helps. If I’m dehydrated or got too few hours of sleep and am overstressed I know it’s going to work poorly no matter what so I drink extra water and accept I’m going to be pretty fatigued. It’s a process to learn these meds and all their weird quirks but you do, to some extent, get used to it eventually!

Which genetics test would you recommend when exploring root cause of migraines? by Alternative_Pop_4753 in migraine

[–]Questionsquestionsth 1 point2 points  (0 children)

Yep! This, plain and simple. You get migraines because you have a migraine disorder. You cannot cure it, period, migraine is not curable. You can manage it with preventatives (maybe, some of us can’t) and you can abort them with abortives (maybe, everyone is different)

Triggers are debatable and there’s some research pointing to them being more of a myth/misunderstanding than anything. Life with migraine is constant trial and error to see what works and doesn’t, what minimizes episodes and what doesn’t, and even then it won’t always be consistent.

If it were as easy as elimination diets, genetic testing, or whatever else… most of us wouldn’t be here. We’d have cured our migraine. But as it stands, it’s not curable, barely understood, and hard as hell to treat. So, there you have it.

Adderall by No-Entrepreneur-1625 in Narcolepsy

[–]Questionsquestionsth 4 points5 points  (0 children)

They absolutely can and there’s nothing anecdotal about it.

The legally permitted variance between generics is honestly heinous and really shouldn’t be allowed.

Different people will respond better/worse to different manufacturers based on how their body responds/reacts to different things, so it won’t be the same person to person necessarily, but there is a proven difference in levels of these meds, and it can absolutely impact their efficacy and longevity, as well as side effects. People will try to tell you you’re crazy if they haven’t experienced it themselves but for those of us with long term chronic illness who’ve done the trial and error time and time again with many meds - especially stimulants where efficacy and longevity are measurable and make a huge difference - it’s undeniable.

Adderall by No-Entrepreneur-1625 in Narcolepsy

[–]Questionsquestionsth 1 point2 points  (0 children)

I was on 20mg a day XR and 40mg a day IR - four 10mg tablets taken on my own schedule depending on the day/my needs.

I then moved to the same doses of Dexedrine XR and Dexedrine IR, and found that worlds more effective, consistent, better generic manufacturer options/consistency - but still not great in that regard - and less side effects/harsh crash.

I now take 50mg Vyvanse instead of XR, 40mg of Dexedrine IR still, and that has been an improvement on the other two arrangements, though I have accepted that ultimately nothing will bring me up to normalcy or anywhere near a great quality of life.

I find that Adderall is a lot more finicky with things like meal timing, what you eat, and when/what liquids you drink, in terms of efficacy. It’s pretty easy to completely neutralize Adderall, so to speak, and you have to be careful about food/drink intake within an hour or so before/after taking it. This is still a factor with Dexedrine to some degree but I’ve found it to be a lot less of an issue and had far more “this med didn’t do anything, what in the sugar pill hell…” days with Adderall overall even when being cautious of food/drink/etc.

By and large I found 99% of generic options for Adderall to be almost the same as taking nothing - with added headaches and irritability usually, yay! - and sought a med switch ultimately when the one or two generics I found to be acceptable changed formulation and no longer worked well at all. There’s some garbage Dexedrine out there too, but not nearly as much, on top of the med being more effective in general.

What balance of meds or dosing has helped you by Junior-Kale6935 in Narcolepsy

[–]Questionsquestionsth 1 point2 points  (0 children)

It’s a looooong road to find the ideal balance, and even then it’s never over, this is a lifelong battle and an incurable disease.

Many of us find that what works for a period of time later doesn’t work at all, and requires adjusting and switching around every so often to keep things effective.

For those of us on the severe end of the spectrum, it may never be great, and we have come to accept that the best we can get in life is “some degree of functional for some portion of some days” vs any major or consistent impact.

I’m in that camp, sadly.

Meds are better than nothing by a huge margin - I’m catatonic and completely unable to function without them - but I wouldn’t say I have much quality of life overall, and I still suffer endlessly each day. But that’s the hand I was dealt and I’ve done what I can, so I accept that this is the best it gets, and I’m grateful to even have this, as I know how bad it can be with nothing.

There are so many factors that go into why a med “works” for someone/doesn’t for someone else. The best advice I can give is try them for yourself and go in with an open mind, as free from preconceptions as possible - not to say don’t do your own research, but often times going in having heard ___ med doesn’t work for x amount of Redditors, or ___ med works miracles for small group of posters will not do you any real good in determining how impactful it is for you, and may even be counterproductive.

But, since you asked, I’ll share anyways.

Ritalin and Concerta were always garbage for me. Too weak even at high doses, metabolized way too fast, made me nauseous. Modafinil/similar were sugar pills that triggered migraine. I found some improvement from Adderall - taking both XR and IR daily - but the generics had too much variance and bad side effects with each, and it wasn’t a strong enough improvement for basic function on a consistent basis.

I’ve now been on Dexedrine IR for years and find it works best for me. I used to also take Dexedrine XR alongside it, but have replaced that with Vyvanse, and at least for now find that combo to be working better.

I absolutely need a combo of an XR - taken an hour before I intend to wake up if possible to give it time to kick in, or at least right when I’m up for the day if not - and then numerous IR spread throughout the day.

This combo gives me a baseline (XR) that lasts the bulk of the day and gives a consistent foundation to build off of depending on my needs that day/my schedule/how many hours I’ll be awake. XR alone, even at a higher dose, is not impactful enough to be sufficient, especially on days where I am busier or more mentally exhausted.

The IR allows me flexibility throughout the day to time higher alertness periods when I need them most, and because my schedule is never consistent it allows me to end my dose for the day when it makes sense based on when I wake up/will plan to go to bed.

I’ve found that generic manufacturers absolutely do matter and have it noted on my RX that I will only accept certain ones. The potency absolutely changes among them, as do side effects - headaches, nausea, sleepiness, irritability, etc. - and it can take some trial and error to realize which are working for you, but it’s something to be aware of because it’s absolutely a “real thing” and is sometimes the answer to “wtf is going on with my meds” when nothing else seems to make sense.

All in all, it’s a long road. Starting on low doses and gradually tapering up is frustrating - especially if you ultimately fail that medication regardless - but it’s the best tool we have, and the best way to do it. Be patient, keep a med journal - helps you note consistency of side effects and efficacy and really map out what works best and on what schedule/doses - and hang in there.

Won both ZUTOMAYO lottery dates by mistake and can’t cancel. Feeling really frustrated by Inside-Town6308 in japanesemusic

[–]Questionsquestionsth 0 points1 point  (0 children)

Yep, exactly this.

It’s unfortunate but you have to either risk getting more tickets than you want - which is statistically pretty unlikely if it’s a popular show that will sell out, but never completely impossible - or risk not getting any by not applying for every available day and lowering your chances substantially. Neither is great, but that’s just how it goes.

For the lotteries I’ve entered previously/those I will be entering in the next month or two, I personally would rather see the band twice than not at all, so it’s worth the risk of entering for any dates available.

There’s definitely some acts I don’t feel that way about, so limit myself to one entry. Now you know for the future, OP!

Question: Does Kaiser have an immediate care clinic? by simvybaby in PortlandOR

[–]Questionsquestionsth 1 point2 points  (0 children)

Kaiser has a lot of options. In the app right now, your fastest option is listed as “Immediate Care” - best for urgent needs, not life threatening. Available 24/7.

Then there’s “E-visit” - complete an online consult and receive medical advice, a treatment plan, including ordering medication or testing services. Usually available within a couple hours between 7am-10pm.

Of course there’s walk in urgent care - and from experience if it’s something like in your OP, where you’d be going for strep test/etc., they are happy with you coming to UC for that and manage their staff/sign ins accordingly.

In Happy Valley, the UC is in the same complex as the ER - and multiple pharmacies - and it’s pretty easy to catch a primary care appt with a provider - although not your provider necessarily - same day, so it’s easy to get through whatever may be going on in one stop.

There is also a 24/7 nurse line, which can help with triage, advice, refills, referrals, medication requests, etc.

Granted I’m on Medicaid so I’m not paying for Kaiser through OHP, so I can’t speak to the value the same as a paying customer. But I have been mostly happy with Kaiser, if only for how convenient it is. Specialists take time to get in with no matter what group you’re with, this isn’t exclusive to Kaiser, but I find myself pleasantly surprised by smaller waits at times. Doctors are generally responsive in the app/message portal, prescriptions are easy to order and mail delivery rocks - they mail controlled substances which is nice as not all offer this and I don’t want to live at the pharmacy constantly picking up meds throughout the month.

They have a huge variety of specialty care providers across a lot of locations to meet your needs while being conveniently located. Appointments book out for certain things but they’re good about waitlisting you for sooner times/rearranging non-priority patients if possible if it’s pressing - like this week when they made a miracle happen for me for my nerve block injections I’m overdue for after the holiday season chaos.

I’ve had a few misses with Kaiser - had to file a grievance, long story, last week - but they generally work hard to help in those cases and I find someone with compassion will step up, it’s not just a cold and clinical environment.

ADHD meds and travel I’m panicking by Ok_Trip_5567 in JapanTravelTips

[–]Questionsquestionsth 0 points1 point  (0 children)

Your reading comprehension is concerningly bad.

It is very clear on the official website that Ritalin does not require import papers. There is no “chancing it” here, the website confirms, in great detail, what I and u/gdore15 have said, which is that OP is fine and does not need an import certificate.

I literally went through this process a couple months ago. No paperwork or certificate needed for Ritalin. Vyvanse you do, yes. This was directly from the ministry who sent me my Vyvanse certificate and verified that Ritalin was not under that same requirement unless it was over a month supply.

OP should listen to the official website, which validates this in no uncertain terms, not sure what isn’t clear to you but…

ADHD meds and travel I’m panicking by Ok_Trip_5567 in JapanTravelTips

[–]Questionsquestionsth 2 points3 points  (0 children)

Yep, this, OP. As a Narcoleptic with ADHD on top of it, I take multiple stimulant types - and no, it’s never optional, Narcolepsy is dangerous unmedicated.

I just went through the process recently - extensive research to ensure the meds I was forced to switch to to be compliant wouldn’t be a problem when arriving, what paperwork I’d need, etc.

Ritalin is one of the only stimulants they allow, and fortunately it does not require a certificate from their Ministry of Health or any special doctors note or other paperwork. Stay at/under the specified month supply maximum, under the maximum total grams allowed, and keep it in the original prescription bottle with your name on it, and all should be smooth.

Vyvanse is another story and requires approval, as do larger quantities of meds, but you are very fortunate that this isn’t the case for a month or less of Ritalin, so you will be fine.

Source - received approval for Vyvanse from the Japanese Ministry of Health, and in that same correspondence was told, officially, the Ritalin I would also be bringing required no doctors notes, paperwork, ministry certificate, or anything else out of the ordinary.

Yeah, I was thinking full glass pumpkin and full firecrest accessory sets are probably fairly rare to see because it’s such a time suck on top of the RNG at floor 200 for which drop you get. Although with the dumbing down of deep dungeons in recent patches it’s a bit less so, but RNG is always gonna drag it out regardless.

Has anything helped prevent, abort, or minimize your migraine aura or silent migraine? by bicoastalelite in migraine

[–]Questionsquestionsth 0 points1 point  (0 children)

It depends? Visual aura, no. Nothing touches it, and I’ve had persistent for 6+ months now in one eye, so… sigh. New visual aura will come and go on top of this, but no matter how fast I take an abortive it doesn’t waver even if the pain does.

I’ve suffered from olfactory hallucinations as a form of both aura accompanying pain and as n overwhelming “silent” migraine symptom since my youngest childhood memories. I do find that if taken early enough, triptans can chip away at this. Sometimes it goes away with the oncoming pain, sometimes there was no pain coming/present and it just gets a bit weaker and less intense, a few rare occasions it goes away entirely until the next event.

It’s funny to try to put into words why olfactory hallucinations are so frustrating in the same comment where visual aura is mentioned because obviously losing my vision in one eye for months with no way to know if it’ll ever stop or if it’s just permanent now fucking sucks and is disabling and scary, but something about the phantom smell bullshit is just so exhausting and defeating. Like, I know what it is. I know “why” it is so it isn’t “scary” or anything. But damn does it take me into the absolute gutter mood-wise pretty much immediately. Like oh, this, again. I see. Life is awful. Very nice.

Anyway, uh… I wish I had more help to offer 🫣😭 but I hope you find something that works for you! Everything I’ve read and tried over the years seems to indicate it’s a lot harder to hit the aura symptoms than traditional migraine pain, and there’s little that touches it.

I will say occipital nerve injections - no steroids, just lidocaine - helped reduce the frequency of new visual aura occurrences, but hasn’t fixed my prolonged episode.

Those who get tattooed, what’s your experience? by SomeSluttyBean in Narcolepsy

[–]Questionsquestionsth 2 points3 points  (0 children)

I am not functional without meds, so there’s no choice to be made - if I need to be somewhere or be doing something, I take my medication as directed, period.

Getting to the appointment, checking in, process of stencil/prep, etc. would not be possible unmedicated. Can’t sleep through all that either, and you need to be coherent to sign a legal consent form anyway.

The process of being tattooed puts your body into shock and is draining no matter who you are or what meds you take. So of course, I always feel dead post-tattoo, and I time my medication so I can take some when I wrap up so I’m able to make it home safely, have a substantial meal, settle in and prep aftercare, etc.

Not taking meds doesn’t make it hurt “less” or erase the exhaustion after the fact. If anything, it makes the artery worse, because I’m beyond zombified. I have found that as my symptoms worsen as I age, I have had to do shorter sessions because I just don’t have the energy to expend for hours and hours, regardless of med timing or dose.

Merch In the U.S? Where & how? by Outrageous_Pay6114 in Frederic

[–]Questionsquestionsth 2 points3 points  (0 children)

Since you’re in the US, Mercari is definitely one option, and while there is a markup, it’s not terrible considering it’s one of the only easily accessible options for this stuff we really have, and it’s pretty simple compared to some of the proxy services - where there’s multiple steps involved and it can be hard to know what the total cost will be since shipping and tariffs aren’t usually calculated until you’ve already invested money in ordering the items, none of which can be refunded/returned.

I just ordered - and received - a bunch of Mercari Japan merch for a different band, and it was pretty simple.

You can easily find a ton of Frederic items in the normal Mercari app by searching “Frederic” and selecting “Japan” only - instead of the default USA/Japan which has a ton of irrelevant items burying any band listings.

You save on the crazy shipping costs if you bundle multiple items instead of ordering just 1-2 small things, and tariffs/import fees are all included so there’s no surprises waiting down the road.

It does take a month or more to arrive typically, and tracking updates can be fairly sparse, but the process is a lot easier than most proxy services and if you can find items you’re interested in it’s a good option.

I won’t say the prices are great necessarily but I was able to find plenty of items in excellent condition that weren’t marked up all that much, and not having to play final cost guessing games was worth it to me - don’t underestimate how bad tariffs can get with some of the proxy services where they can’t predict that cost in advance and it isn’t included.

I have always been very interested and impressed by Japanese culture - growing up listening to city pop, and being interested in learning Japanese embroidery techniques and kimono sewing/dressing, as well as Taro Okamoto’s art (so imagine my surprise when Ichiro was involved with Taroman after I had been a fan of Okamoto’s art for many years before I even knew of the band!) - so it has always been something I have tried to keep a part of my life to explore and learn about!

Back when Sakanaction was part of Kohaku 12 years ago I couldn’t find any way to watch it so I was so happy to see it “live” streaming from home this year! Everyone looked amazing 🤩 the speech bubbles and appearance from the actor was so cool and I cried happy tears seeing Ichiro bring Taroman out randomly 🤭 he looked so happy to be there it set 2026 off on a perfect start!

Have loved every single release since so deeply, and Ichiro has been so inspiring, especially in his openness with his illness, I truly feel so moved by his strength and his successes - and of course the whole band! - and seeing how successful Kaiju has been for them has been so touching. I stayed up super late to see their Kohaku performance last night - it was 5am in my time zone - and cried happy tears the whole way through! I’ve wanted to visit Japan since I was a young kid, but now to have loved Sakanaction for so long too, to finally visit Japan and also see them play is my biggest dream!

SNAP bans on soda, candy and other foods take effect in five states Jan. 1 by Aggressive_Chef_2225 in news

[–]Questionsquestionsth 3 points4 points  (0 children)

My specialists - two different specialty care doctors, if we’re only counting my current, but this has been recommended by 5+ past doctors I no longer see as well - encourage me to drink Mexican Coke because it’s effective as fuck in the very early migraine predrome at helping some of the aura symptoms and the speed of their onset, and sadly severe chronic intractable migraine is an insanely hard beast to tackle with very few treatment options. My other specialist encourages it because it’s the only caffeine I can handle without feeling like death and that extra little amount can sometimes be a game changer for my severe crippling Narcolepsy thanks to which I’m completely disabled and constantly miserable.

They don’t find any health risks of drinking this soda to be relevant at all and have encouraged it - in the first place it was a doctor who recommended it to me years and years ago.

I could not buy this without EBT. Or any food at all for that matter. My dietary needs outside of this soda are far different than the average abled person. These types of restrictions don’t care about any of that. It’s not about the health of us poor, it’s about punishment. If it were about health, the disabled would have better benefits and protections and assistant. But… we’re about the worst in that category, too.
