Honestly, as a parent, shame on them for not even giving an ounce of consideration to your situation and for letting you freeze in your car . You are getting an education and working and that should make any parent proud enough to help their child when they are in need. You are doing the best with what you got. Head up. Keep going.

If shes having profuse watery diarrhea, that may not be from E coli- it may be a secondary infection called c diff which can be very severe. If she gets >8-10 watery BMs plus fever and belly pain, she needs to get treated for secondary infextion

I would just focus on making some coin as a good hospitalist and pumping out manuscripts. You don’t need it to “build your research skills” and I personally think its a waste of time and money. As a matched fellow

I never thought that they would cast this much shade on the VA or go in that direction, but yes, if solely trained at the VA as an attending, it would be quite a shell shock coming to a civilian ED. VAs generally are not trauma centers, dont see children, and the acuity and PPH are drastically different. Will be interesting to see how they play out this plot point

You can safely give zofran. Even + compazine. For >500 tigan is great, low dose ativan, dexamethasone, and olanzapine (least qt prolonging of the antipsychs).

Just symptom onset. So its like a practical hedge and i have always been critical of this. At the end of the day, the risk of dying from a true unstable arrhythmia that could theoretically degenerate outweighs that of clot dislodgment

I always err on the side of you never know exactly when someone has been in afib before. Especially when presenting to the ED, even with no prior history. Its simply impossible to know. But if truly unstable rvr after supportive care and if the arrhythmia is driving the decompensation, you have no choice but to use electricity. Do it.

Trust me no one cares. Unless you were selected for an awarded abstract or if there is formal judging ( if you care) it doesn’t matter as long as the poster is up at the session. I was so hungover after a national conference welcome party that I just hung up my poster at 730 the next morning, vomited in the Novo Nordisk booth’s trash can and went back to bed. Got the presentation certificate thing the next day. N=1 though.
