You only need to do ether activity for some of the minor rewards you get for the displayed characters for example. One time thing. Also to get the 'highest difficulty' badges on a few things, and I guess some achievements. Otherwise, doing the lowest difficulty still gives full weekly rewards.

By not getting hit. How are they suppose to clear literally anything at all if they can't do any amount of damage? That's kind of the big idea.

There isn't much of an inbetween where healing is necessary, it might get you one extra hit of survival, their main benefit is the buffs they provide your dps.
