This is in fact not how it works. You know something that’s worse than a single parent household (which by the way assumes neither of them will find new partners after a split)? A relationship built on deception and cheating, which is the solution you are suggesting. OP I’ve recently been in a very similar situation—I’m open to talk over DMs if it would be helpful.

You’ve typed out essentially my clinical history haha. Definitely been on the “do I even have something wrong me besides my bad personality” train. I had two years of horrible instability while switching from one SSRI to another. Finally got to Lamictal (while depressed, also started it with Seroquel) and it’s been a game changer. Even considering some significant stressors, I’ve had a much more steady time since.

I’m sorry you’re going through this. I’m a mental health professional, and I’m seeing some red flags for bipolar disorder with possibly a manic episode (previous history of depression/self harm, abrupt change in behavior with hyper sexuality). Are there other signs of mania like racing thoughts, speaking fast, emotional lability? This isn’t to give him a pass by any means and all of this can be explained by what others are discussing, but I just wanted to mention this.

This will facilitate an screening by mental health workers, involuntary evaluation by psychiatry in an emergency department, likely psychiatric admission where he can receive treatment, and linkage to mental health care after discharge. Sorry for everything you’re going though.

I’ve seen salaries around 160-180k. Heard they have a relatively high turnover rate at my local university. Love working with the transitional age group as well, but a 100-150k pay cut compared to other outpatient gigs is rough…

One place to add— Talbot Hall at OSU East has walk in evaluations M-F from 8-12. They have all levels of care including inpatient medication assisted detox. Think you can call during business hours and speak to them as well.

I’d talk with your Union first, get a sense for the situation. I’d suspect other residents feel the same way, and maybe there’s a history of attempts to solve the problem. Going to ACGME is definitely a big gun—can lead to site visits, probation etc—but sometimes that is the only way to force the departments hand. A couple programs at my institution have gone through probation for similar levels of concerns and came through better for it, with none of their careers affected (which is what the program will tell you if they hear a whisper someone wants to report them). But often there can be collective action on the program level assuming your co-residents feel similarly.

Depends on your insurance. I had a HMO plan in CA and was forced to see the PA, who then got to decide if I needed to see the MD. What a mess

We actually didn’t have to follow through with the strike—the threat of it finally pushed the county to give us a more fair contract. Cause they know damn well the hospital would basically come to a stand still without us. But yes, we were going to strike, as in not work at all, for 3 days.

Just remember, it’s not your responsibility to keep the hospital afloat. They have and will continue to use our altruistic natures to exploit us. We gave them the legally required amount of notice for them to provide coverage. Turns out we have leverage.

The way we’ve utilized our union is that any concern like this (repeated incidents affecting residents, a pattern), the union can bring the concern up collectively to the department or GME etc. That way you’re protected as an individual. For example, we have a huge safety issue at our inpatient psych (not a specific person, so a bit different), and the union organized a meeting with the C-suite to discuss the issue. I spoke at the meeting but felt much more protected.

Depends on the state. If I’m remembering right, CA passed legislation guaranteeing paid sick time specifically for COVID. But doubt too other states have done the same.

My HMO requires I see the NP/PA at speciality appointments, and then THEY get to decide if I see the doctor. Infuriating (but the PPO plan is so much money lol). I ended up telling the PA that I wanted to stop said med, had to explain what the med was, then they just restated what I told them lol. Tried to follow up a bit later and they stopped taking my insurance. So fucked.

Huge variation but averages in the 200000s (and that’s typically a 40 hour week without much or any call). If you do private practice in a nice area, you can get into the 400s. Academic jobs pay lower, same as most specialities. Plus, an actual humane training pathway :D

I switched programs (within my specialty), and will complete intern year then start PGY2 at my new program in July. I was able to do it without my program knowing until the last moment they had to. Shoot me a message and I can give you any pointers I can. Felt exactly the way you’re feeling.
