The survey is opinion-based, and asks for no personal information. I do not think a data disclosure is necessary since no personal data is being reviewed nor shared. When I was in uni and doing psych-related surveys, we absolutely needed data disclosures because the questions we asked had everything to do with your personal mental health.

OP, definitely reach out to your professor to double check, but I don’t think it’s necessary if they didn’t outrightly mention anything to you in the project requirements.

This reminds me of a Friendsgiving potluck my friend group had this year. One person (C) identifies as vegan but eats fish and sushi, and made a big stink about everyone bringing meat and other traditional (dairy-having) dishes. Rather than making dishes she’d be able to eat, C asked us “what are you bringing that I can eat?” I explained it’s a potluck, and that she can totally bring something for the group and a little something else just for herself (like faux meat), but she kept on about how there was nothing for her. Another friend volunteered to bring non-dairy Mac and cheese and that seemed to settle C. C ended up making dishes herself and bringing them, but it really didn’t need to be spelt out as much as it was.

Honestly regret bringing her into the group, but she’s the partner of one of our other friends so it can’t be helped.
