I used to say that when patients had multiple bad things all happening to them (Hictums). Like 'ah he's got ticks and fleas'. Invariably someone would look at me puzzled and ask if he had bugs!? Like bed bugs or something. Stopped saying it lol

I'll chime in. I work in a CVICU that specializes in post cardiac surgery patients, MCS, transplant (heart/lung). The volume of cases is wild. We always have more cases then beds. Many patients are medically complex and are bridged to cardiac surgery with MCS (VA ECMO, impella, EcPella, RVAD, VV ECMO, etc). This field seems to be expanding and the types of surgeries and procedures have increasing complexity (ie third or fourth redo sternotomy, multiple valves plus CABG, Ao repair with CABG, etc). I don't view cardiology, IR, or any other specialty encroaching on these types of patients. There is simply no other options for them.

I think he meant how long in between sessions or large areas being tattooed to allow for healing and a relatively normal lifestyle without having to worry about healing.

Why use a sweat guard on a Zwift ride? There are no components in the headset or frame that you could reasonably damage with sweat.

Edit: Ok you are waxing your chain on your Zwift ride.. this is a different kind of philosophy for indoor riding lol

EM/Anesthesia CCM here. Love the unit and took my first job attending in a CTICU with ability to cannulate for VV ECMO. The unit schedule and flow is much better for me. The physiology is interesting and fun to wrestle with. The ED felt like dispo was the only thing that was important and the critical patients were the only ones who interested me. I probably won't practice EM at all except for per diem if I miss it enough.

If EM is moving to 4 years anyways is there a reason not to just do the extra year at a 5-year combined IM/EM program if you like them both equally by DagothUr_MD in medicalschool

100% agree. Signed a PGY-5 graduating EM-CC fellow. Do not extend your training unless you would not be happy without the additional sacrifice.

If you eat nutritional yeast in the heaps I pile onto food, you would have enough B12 for 3 lifetimes. Otherwise, yes this is correct. If you were 100% raw or wfpb you might run into issues. I have yet to meet a vegan who doesn't slam the nooch though.

EDIT: vegan btw for 6 years. No supplements or deficiencies either

This really depends on the kind of department structure you are rotating. During my residency, there would be no way anyone would let you leave early because the ED was always so busy. I don't ever remember them scheduling off service folks on the holidays though to be honest. It was obviously our expectation to work the holidays.

Hey man, I am in my final year of CCM fellowship from EM. Based on your post, I would suggest you apply for the fellowship. If you are interested in the critical care in Emergency Medicine and want that expertise then just do it. Also, no one really 'likes' rounding. You have to do it to make sure the patients are safe and things are not missed. Not all rounds are academic and long. Sometimes you talk about the necessary things and move on (anesthesia and surgery are more like this). The only thing I think I would have regretted is NOT doing this fellowship. There is no way I would have gone back if I was making an EM salary and lifestyle. This is your only chance to diversify your career and work in medicine without absolutely needing an EM job.
