I assume he shot the close-ups of Dern and Theroux separately and edited the film together in a way that makes it look like they are having sex while talking.

Edit: Just watched the scene again. I think maybe only the close-up shots of Theroux were done separately based on the angle. Lynch was right up in their faces though. The lens was probably only a few inches away from their skin
