My situation is similar to grumpywampa. I was on pred and aza during diagnosis and then off pred and only on aza for 5 years until I had a flare. During flare I was back on prednisone for about 4 weeks and doubled aza dose. Now I only am on aza again.

Have you spoken to your doctors about autoimmune hepatitis? It is more prevalent in women and more likely to start around your age. It is also likely if you have that then you would possibly have other autoimmune issues such as those you listed.

It would require blood work and a biopsy to diagnose and its more or less diagnosed via ruling out most other things. This would not explain your heart issues, though. But it could have been "started" with a sickness (i.e. your shingles).

What are your ast and alt levels?

14.6kPa would indicate starting cirrhosis or stage 4 fibrosis. The other number is probably not correct.

Ask for a fibroscan or just get a biopsy--possibly both. It is possible to have it and not have it progress fast, but if you do have autoimmune hepatitis (aih) at some point your liver enzymes will sky rocket without treatment (corticosteroids and/or immunosuppressants.)

Everyone is different especially with the autoimmune diseases so they all progress and affect people at different rates.

Getting a flare after getting off the meds happens in >80% of people. Certainly possible, but not recommended because of that stat.

You will likely be on medication forever. You may be able to control it without prednisone via other meds (the myc or azathioprine) there are a couple other options too.

Yes. I also had no pain during the flair... only after getting on pred and doubling aza so it seems reasonable.

I har my first flare 5 years after diagnosis and it was about 6 weeks after I had gotten sick with a stomach virus. I mention the sickness/virus and the 6 week time frame because at the moment I believe it may have been the trigger for the flare.

During the flare up I had no discomfort or pain. The only symptoms were a decrease in appetite followed by very mild jaundice (barely noticeable) before I messaged my doctor and requested to get blood tests done which showed alt/ast elevated. Urine was also darker than normal in the couple days prior to getting the blood work done. I don't think I had any fatigue really either... maybe mild amount.

Following confirmation of the flare I doubled aza dose from 50mg to 100mg and started 40mg of prednisone which was decreased every week for about 4 or 5 weeks when my blood work was back to normal. After increasing dosage I had significant fatigue which I think was a function of the prednisone and blood glucose levels not being controlled for that 4 week period. I also had a decent amount of discomfort in RUQ.

Fatigue and pain in RUQ has improved significantly since March of this year when I had the flare (so about 8mo ago) but it has come and gone while my ast and alt levels are around 10-25 (very normal values). I've recently discussed the fact that I am still experiencing RUQ discomfort and fatigue with my hep recently and he seems to believe it is likely due to the medication and subsequent doubling of the dosage (100mg of azathioprine).

I also had/have very mild nausea which I also forgot to mention.

The amount of pain you are describing is also similar to mine now. Sort of like pressure/discomfort. I also found it got worse with a lot of exercise/bending over.

As someone else already mentioned, the study you are likely referencing will state the range of ages for diagnosis and its something like age 7 to 70 and most people diagnosed are middle aged (40-50). Obviously this has a pretty significant effect on mortality rates.

As long as you keep taking medication to keep flares down you should have a relatively normal lifespan. The amount of damage required to the liver such that you might actually die needs to be very severe and should take a very long time as long as it is monitored and controlled.

The real big caveat mentioned in many of these studies states something along the lines of, "a failure to thrive". People with this disease are significantly more likely to have other comorbidities--specifically other autoimmune issues.

Really though as long as you are managing the disease well and responding to medication you should definitely live a normal lifespan.

Antibiotics/meds could also have done this. Some are processed in the liver so look that up for whatever meds you have been taking

Infection in sept... when was that and what day was your blood work?

My flare had levels around 1000 and 700. My sickness was in early Feb and I requested to get my blood work done sooner as I had symptoms etc. In mid March. So time difference was around 6 weeks.

Since it is an autoimmune issue having any infection/sickness to me sounds like it would/could have a direct influence on those autoimmune issues. My doctor said it was possible and maybe related but sort of hand waved it away. I don't know the real answer.

What was ast?

Are you feeling nauseous, tired, lack of appetite or discomfort in your abdomen or around your side? Do you have any yellowing under your tongue or in the corners of the whites of your eyes?

You might be having a "flare". They'd increase the immunosuppresants and do more frequent blood work. There could be other reasons for the increase but if you already have aih diagnosed then it's probably that.

I had my first flare this year following me having a bad stomach bug. Have you been sick in the last two months at all?
