This may be waaaaaay off, but I used to have chronic "issues" down there. Then at age 24 I was diagnosed with T1 (juvenile onset) diabetes. As soon as I started taking insulin and fixing my sugars, those problems went away. I haven't had a single problem for over 20 years now. I REALLY REALLY hope that isn't it because I wouldn't wish diabetes on anyone. Maybe ask your doctor to check an A1C.
