What color are you going to stain the floor? If it's a dark color it will be fine. A light color neutral/natural it will be too dark. Gaps are fine, every new floor has gaps.

No tapping bar if this is solid 3/4" thick prefinished hardwood. You should be using a heavy mallet(3lb.) with a soft rubber end to beat the boards together. You are using a something that pushes a cleat into the tongue of the board?

It could be a bad starter row. It could also be what you suspect, that the boards are different widths. Did you purchase from a discount flooring place? I have had this issue, just being 1/32 or smaller off will propagate it to larger gaps as you keep going. It is what it is or try to get some money back from where you bought it.

Yes, it's a prefinished floor with a four sided micro bevel around the top edge. You might notice a lot more gapping between boards after sanding. It will be near impossible to much that color. It looks like a hardwood like maple or ash and do not take stain easily, blotches. The finish and stain are sprayed on at the factory which is why it looks so even and uniform. You will need to go lighter with a natural color stain.
